Primary Mucinous Adenocarcinoma of the Urinary Bladder with Signet-Ring Cells: Description of an Uncommon Case and Critical Points in Its Management

We present an uncommon case of mucinous adenocarcinoma of the bladder (MAB) with signet-ring cells extensively infiltrating prostate gland and pelvic/retroperitoneal lymph node stations and not responsive to usual systemic chemotherapy regimens. This case highlights the important features of MAB including the pattern of tumor spread, the tendency for initial misdiagnosis, and the importance of immunohistochemical study in order to define its primary origin from the bladder and choose the most appropriate treatment since the beginning.

Fenoterol is a β<sub>2</sub>-adrenoceptor (AR)-selective agonist that is commonly used to investigate relaxation responses mediated by β<sub>2</sub>-AR in smooth muscle preparations. Some data have questioned this because fenoterol had low potency in the rat urinary bladder when a muscarinic agonist was used as a pre-contraction agent and because some investigators proposed that fenoterol may act in part via β<sub>3</sub>-AR. Hermann et al. [1] suggest that minimum quality criteria are needed to communicate unexpected drug safety findings from randomized clinical trials and imply that our report on unexpected frequent hepatotoxicity in patients receiving flupirtine for the treatment of overactive bladder syndrome [2] does not meet such criteria. As the brief report format of our paper did not allow an extensive description of trial details, we welcome this opportunity to shed more light on the matter.

As a preliminary approach to define the object of larger case-control studies, the distribution of potential risk factors coming from environmental pollution among patients affected by superficial bladder cancer (TCCB) has been analyzed. Alpha-adrenergic drugs in retrograde ejaculation.

6 men with complete or partial loss of ejaculation as a result of retroperitoneal lymphadenectomy (performed because of testicular tumor) were treated with an intravenous injection of 60 mg synephrine. Synephrine stimulates adrenergic alpha-receptors. Only 1 man with sustained retrograde ejaculation improved following treatment showing antegrade ejaculation. This temporary restoration of fertility was thought to be due to an increase in bladder neck tone and prevention of backflow of semen into the bladder as a result of stimulation of adrenergic alpha-receptors.
