Abstract Sporadic thoracic aortic aneurysms (TAA) and dissections are one of the major causes of morbidity and mortality worldwide, especially in those older than 65 years. The presentation of TAA is varied and often silent. Thus, sporadic TAA detection is often fortuitous, with identification occurring during a routine physical examination or during an unrelated medical evaluation. Intravascular ultrasound (IVUS) is a promising new technique for assessing vascular morphology and structure. Controversy exists whether the three-layer appearance of the arterial wall correctly reflects the histologic structures of the intima, media, and adventitia. We performed an experimental study to clarify the three-layer appearance. The vessel wall architecture was analyzed by IVUS on eight different kinds of plastic cylinders, 24 normal blood vessels from pigs, and 59 human arterial segments. Ascending aortic aneurysms (AsAA) and aortic dissections are life-threatening cardiovascular diseases. The main criteria for determining surgical intervention of AsAA are the maximum diameter or the increasing annual rate of the aneurysm. The mortality of the untreated aortic dissection can be 21% to 74%, depending on the delay of hospital admission.Our study aims to characterize the biomechanical properties of the ascending aorta and propose a patient-specific approach to assess the risks of rupture.
