Cardiovascular imaging in the diagnosis and monitoring of cardiotoxicity: cardiovascular magnetic resonance and nuclear cardiology
2016
Chemotherapy-induced cardiotoxicity (CTX) is a determining factor for the quality of life and mortality of patients administered potentially cardiotoxic drugs and in long-term cancer survivors. Therefore, prevention and early detection of CTX are highly desirable, as is the exploration of alternative therapeutic strategies and/or the proposal of potentially cardioprotective treatments. In recent years, cardiovascular imaging has acquired a pivotal role in this setting. Although echocardiography remains the diagnostic method most used to monitor cancer patients, the need for more reliable, reproducible and accurate detection of early chemotherapy-induced CTX has encouraged the introduction o…
La fibrilación auricular permanente en las enfermedades cardiovasculares en España. Estudio CARDIOTENS 1999
2002
Introduction and objective. Atrial fibrillation is the most common arrhythmia seen in clinical practice. The objective of this study was to know the frequency of atrial fibrillation and the characteristics of patients with atrial fibrillation in the Cardiotens study. Material and method. A cross-section study with systematic selection of the study sample. All 32,051 outpatients seen on the same day by 1,159 physicians specialized in primary-care (79%) and cardiology (21%) were prospectively added to a database including history of cardiac disease (heart failure, coronary disease or atrial fibrillation), blood pressure, and ongoing treatment. Results. Atrial fibrillation was present in 25% o…
Digoxin-related leukocytoclastic vasculitis in a very elderly woman: A case report
2016
International audience; Even though digoxin causes many side effects, few cases of skin involvement are recorded in the French Pharmacovigilance Database. We report a case of leukocytoclastic vasculitis (LV) very probably due to digoxin. A 91-year-old woman, hospitalized following a fall, presented cardiac decompensation in a context of rapid atrial fibrillation requiring treatment with digoxin. Eight days later, a rash appeared on her back and trunk. It was neither itchy, nor painful and persisted despite local treatment. There were no other clinical anomalies. After a few days, the rash spread with appearance of bullous lesions, ulcerations and a necrosis on lymphedema of the two legs. Am…
Subclinical atrial fibrillation: how hard should we look?
2012
More than three decades ago, an analysis from the Framingham Heart Study revealed that atrial fibrillation (AF) increases the risk of stroke by a factor of five in non-rheumatic AF and by a factor of 17 in rheumatic AF.1 Since then, it has convincingly been shown that anticoagulation is one of the most effective secondary stroke prophylactic treatment options, which reduces the risk of stroke by 2/3,2 even in an older population.3 AF may occur in different types and in an individual patient often starts with paroxysmal AF, but later becomes persistent or permanent AF.4 Interestingly, the risk of stroke or systemic embolism is influenced by cardiovascular risk factors such as hypertension, d…
Fibrates in primary prevention of cardiovascular disease. Comments on the results of a systematic review of the Cochrane Collaboration
2018
Abstract Fibrates are drugs that reduce triglycerides, elevate high-density lipoproteins, as well as decrease small, dense LDL particles. The results of a study have recently been published by the Cochrane Collaboration on fibrates efficacy and safety in the primary prevention of cardiovascular disease. This study includes a systematic review and a meta-analysis of 6 studies (16,135 patients) that evaluated the clinical benefits of fibrates compared to placebo use or other lipid-lowering drugs. This review showed evidence of a protective effect of the fibrates compared with placebo as regards a reduction 16% of a compound objective of death due to cardiovascular disease, non-fatal myocardia…
EpCAM and microvascular obstruction in patients with STEMI: a cardiac magnetic resonance study
2021
Abstract Funding Acknowledgements Type of funding sources: Public grant(s) – EU funding. Main funding source(s): This study was funded by “Instituto de Salud Carlos III” and “Fondos Europeos de Desarrollo Regional FEDER” Bachground. Microvascular obstruction (MVO) is negatively associated with cardiac structure and worse prognosis after ST-segment elevation myocardial infarction (STEMI). Epithelial cell adhesion molecule (EpCAM), involved in endothelium adhesion, is an understudied area in the MVO setting. Purpose. We aimed to evaluate whether EpCAM is associated with the appearance of cardiac magnetic resonance (CMR)-derived MVO and long-term systolic function in reperfused STEMI. Methods.…
Heart failure with preserved ejection fraction infrequently evolves toward a reduced phenotype in long-term survivors: a long-term prospective longit…
2019
Background: Long-term trajectories of left ventricular ejection fraction (LVEF) in heart failure (HF) patients with preserved EF (HFpEF) remain unclear. Our objective was to assess long-term longitudinal trajectories in consecutive HFpEF patients and the prognostic impact of LVEF dynamic changes over time. Methods and Results: Consecutive ambulatory HFpEF patients admitted to a multidisciplinary HF Unit were prospectively evaluated by 2-dimensional echocardiography at baseline and at 1, 3, 5, 7, 9, and 11 years of follow-up. Exclusion criteria were patients having a previous known LVEF <50%, patients undergoing only 1 echocardiogram study, and those with a diagnosis of dilated, noncompa…
Symmetric dimethylarginine serum level as a new marker of left ventricular ejection fraction in patients with acute myocardial infarction
2013
Purpose: Asymmetric dimethylarginine (ADMA) is a by-product of protein methylation that has been implicated in the prognosis after acute myocardial infarction (MI) and heart failure through Nitric Oxide Synthase (NOS) inhibition. We aimed to investigate whether SDMA – the endogenous symmetrical stereoisomer of ADMA- that has insignificant inhibitory effects on NOS- might be a marker of left ventricular function in acute MI. Methods: Blood samples from 635 consecutive patients hospitalized 1 (23%). Mean LVEF was 55±13%. Mean ADMA, SDMA and L-arg levels were at 0.72±0.42, 0.51±0.44 and 91±54 μmol/L, respectively. Spearman analysis showed that LVEF was correlated negatively with SDMA (r=-0.151…
Effects of Two Different ACE Inhibitors, Captopril and Delapril, in the Early Phases of Acute Anterior Myocardial Infarction
1999
Objective: This study was aimed at investigating the effects of captopril or delapril before and just after thrombolysis in acute anterior myocardial infarction (AMI). Patients and Methods: Study participants consisted of 89 patients, hospitalised within 4 hours of the onset of symptoms. Patients were randomly divided into two groups: the first group (45 patients, pretreatment group) received captopril 6.25mg or delapril 7.5mg orally about 15 minutes before intravenous administration of recombinant tissue plasminogen activator (rt-PA) 100mg; the second group (44 patients, late-treatment group) received captopril or delapril about 4 to 6 hours after thrombolytic treatment. Captopril/delapril…
0058 : Low circulating levels of Growth Differentiation Factor 15 (GDF-15) before coronary artery bypass surgery may predict post-operative atrial fi…
2015
BackgroundPost-operative atrial fibrillation (POAF) following cardiac surgery has a high prevalence and is associated with considerable morbidity.AimsTo assess the role of GDF-15, a member of the TGFβsuperfamily, as a potential new predictor of POAF after off-pump (OFP) and on-pump (ONP) coronary artery bypass graft (CABG) surgery.MethodsThis observational study recruited the first 50 patients planned for OFP surgery and the first 50 patients planned for ONP surgery. Patients referred for CABG with the following exclusion criteria: age 80 years, previous atrial fibrillation/flutter, previous treatment with Amiodarone, previous cardiac surgery, emergency surgery. Included patients were equip…
