Neutrophil Adherence to Activated Saphenous Vein and Mammary Endothelium After Graft Preparation
2005
Interaction of circulating leukocytes and vascular endothelium plays an important role in vasoconstriction, endothelial dysfunction, and vascular injury. Dilation procedures of grafts before coronary artery bypass graft surgery might lead to vascular injury and subsequent bypass graft disease. We analyzed in vitro the adherence of fluorescence-labeled polymorphonuclear neutrophils (PMNs) to endothelium of human saphenous vein grafts or internal mammary artery grafts after stimulation with thrombin (0.5 to 2 U/mL) or dilating procedures. Furthermore, we investigated endothelial function of prepared grafts.Thrombin stimulation resulted in a dose-dependent increase of PMN adherence to the endot…
A review of 507 off-pump coronary bypass patients: a single center experience.
2007
This retrospective study evaluates perioperative results of 'Off-pump' coronary artery bypass surgery (OPCAB) experience in a single center. Five hundred and seven patients were operated (median sternotomy) from 1998 to 2002 using OPCAB. Patient data were registered and risk prediction calculated using the full logistic version of EuroSCORE. Overall, 1091 distal anastomoses were performed and only five (1%) patients required conversion to cardiopulmonary bypass. The predicted mortality was 3.8+/-4.5%, the observed mortality was 2.37%. OPCAB technique has become a standard approach in our department with low mortality and morbidity rates even in treatment of multivessel disease or high-risk …
Proliferative activity in stenotic human aortocoronary bypass grafts
2002
Abstract Background: Aortocoronary bypass graft disease is responsible for long-term failure of autologous vein grafts. The analyses of proliferation and cell type characterisation in human bypass grafts harvested during re-do surgery make it possible to investigate the cellular processes leading to bypass graft failure. Methods: 30 stenotic vein grafts and 25 control veins were explantated during re-do heart surgery procedures. The total area and cell count of the neointima, media and adventitia were calculated computer-assisted. Actively proliferating cells were identified using antibody to Ki-67 and positive cells were determined by double-label immunocytochemistry with SMC α-actin, CD 3…
Electrophysiologic evaluation of phrenic nerve and diaphragm function after coronary bypass surgery: Prospective study of diabetes and other risk fac…
2006
Objective Phrenic neuropathy after coronary artery bypass grafting has been related to various risk factors with conflicting results. The aim of this study was to assess the incidence, characteristics, and clinical consequences of phrenic neuropathy and the influence of diabetes and other risk factors. Methods We conducted an observational, prospective study of parallel groups including 94 consecutive patients subjected to coronary artery bypass grafting, half of them with diabetes and associated polyneuropathy . Electrophysiologic study of phrenic nerve conduction as the reference method, chest radiography, diaphragm ultrasound, and functional respiratory tests were performed 24 to 48 hour…
Introduction to coronary imaging with 64-slice computed tomography
2005
The aim of this article is to illustrate the main technical improvements in the last generation of 64-row CT scanners and the possible applications in coronary angiography. In particular, we describe the new physical components (X-ray tube-detectors system) and the general scan and reconstruction parameters. We then define the scan protocols for coronary angiography with the new generation of 64-row CT scanners to enable radiologists to perform a CT study on the basis of the diagnostic possibilities.
Management and predictors of clinical events in 75 686 patients with acute myocardial infarction
2022
Background:Although mortality of patients with acute myocardial infarction (MI) has decreased substantially over the last few decades in many countries MI remains a major threat to public health. Aims:To assess the number and outcomes of patients hospitalized for acute MI in Poland in 2018 as well as proportions of patients participating in cardiac rehabilitation and undergoing invasive cardiac procedures following discharge. Methods:We used public databases. We included all patients hospitalized for acute MI in Poland in 2018 and assessed event-free survival along with uptake of invasive cardiac procedures and cardiac rehabilitation and consultations with cardiologists. Results: A total of…
Digitale kommunikasjonsløsninger for samhandling mellom koronar bypassopererte pasienter og helsepersonell på spesialistsykehus etter utskrivelse til…
2016
Masteroppgave i helse- og sosialinformatikk- Universitetet i Agder, 2016 Background In Norway there are expectations of further development and strategic investment in information technology within the health care system. Persons who undergo a coronary artery bypass graft (CABG) surgery are in need of information and guidance related to problems that may arise after they have returned home from hospital. Purpose The purpose of this project was to explore to what extent digital communication solutions might meet the need for information and guidance from health personnel experienced by patients who had undergone a CABG surgery, after discharge from the specialist hospital. Additionally, it w…
Pressor amines in coronary artery bypass grafting procedure with and without the use of extracorporeal circulation
2017
Wstęp: Operacje rewaskularyzacji mięśnia sercowego są najczęściej wykonywanymi procedurami w praktyce kardiochirurgicznej. Rozwój technik pomostowania tętnic wieńcowych bez użycia krążenia pozaustrojowego spowodował, iż modyfikacjom uległy również techniki znieczulenia. W prowadzeniu znieczulenia do zabiegów typu OPCA B anestezjolog musi szczególnie szybko reagować na częste, czasem nieprzewidywalne zmiany sytuacji hemodynamicznej. Cel pracy: Celem pracy było porównanie zastosowania amin presyjnych wśród pacjentów poddanych chirurgicznej rewaskularyzacji mięśnia sercowego z krążeniem pozaustrojowym i bez niego. Materiał i metody: Badanie miało charakter retrospektywny. Badaniem objęto 80 ch…
The effect of off-pump coronary artery bypass on mortality after acute coronary syndrome: A meta-analysis
2012
Background Patients requiring surgical revascularisation for acute coronary syndrome (ACS) form a clinically heterogeneous group ranging from haemodynamic stability to cardiogenic shock. Whilst 'off-pump' revascularisation (OPCAB) is often considered, patient selection and operative timing remain controversial. This study aims to identify whether OPCAB may confer a mortality benefit over ONCAB in revascularisation for ACS. Secondly, we review the impact of OPCAB on completeness of revascularisation (CR) and long-term re-intervention. Methods A systematic literature review identified 9 studies (1 randomised controlled trial) of which 8 fulfilled criteria for meta-analysis. Outcomes for a tot…
The Evolution of Cardiovascular Surgery in Elderly Patient: A Review of Current Options and Outcomes
2014
Due to the increase in average life expectancy and the higher incidence of cardiovascular disease with advancing age, more elderly patients present for cardiac surgery nowadays. Advances in pre- and postoperative care have led to the possibility that an increasing number of elderly patients can be operated on safely and with a satisfactory outcome. Currently, coronary artery bypass surgery, aortic and mitral valve surgery, and major surgery of the aorta are performed in elderly patients. The data available show that most cardiac surgical procedures can be performed in elderly patients with a satisfactory outcome. Nevertheless, the risk for these patients is only acceptable in the absence of…
