The proportion of patients with gallbladder stones suitable for extracorporeal shockwave lithotripsy (ESWL) was analysed prospectively in 200 patients aged 17-76 years (62 males, 138 females) with symptomatic cholecystolithiasis. Criteria for inclusion were clinical symptoms, solitary stones (diameter 10-30 mm) or up to three stones with comparable total volume, contractile gallbladder, no calcification of stones, normal biliary tract anatomy. Evaluation of early and late histamine release by three standard volume substitutes

Following meta-analysis [1], the use of colloidal solutions in intensive care medicine is under discussion. Although polygeline [2] and human albumin are known to cause histamine release, other substances eg hydroxyethyl starch were thought to be without effect. Histamine release depends on the clinical scenario, concomitant drugs, infusion speed, duration and amount of infusion. However, there is no information about their effect regarding time periods longer than immediate reactions. This study investigated the histamine releasing effects of three plasma substitutes administered and observed over a longer period of time, simulating many of the clinical scenarios.

Multiple Lesions, Multiple Measures

This case describes the assessment of FFR in the case of tandem intermediate lesions in the proximal and mid LAD. Hyperemia was induced with intravenous (IV) infusion of Adenosine, which allows pullback measurements. First, with the wire placed distal in the vessel, FFR provides information on whether the sum of the two stenoses causes ischemia. Thereafter, during pullback, the most severe stenosis can be identified as the most relevant pressure drop. Finally, a re-evaluation of FFR is important after PCI: after the first stenosis is treated, the degree of hyperemia achievable will be larger, thus unmasking the relevance of the second one.

Maximal isometric muscle strength and anthropometry in 75-year-old men and women in three Nordic localities

Isometric maximal strength of hand grip, elbow flexion, knee extension, trunk extension and trunk flexion was studied in representative samples of 75-year-old residents of 3 Nordic localities: Glostrup in Denmark, Gothenburg in Sweden and Jyvaskyla in Finland. The study forms a part of a wider comparative research project analyzing the functional capacity and health of 75-year-olds in these towns. The strength measurements were carried out using standardized techniques and identical dynamometers. Lithotripsy of gallbladder stones in 1992: Improved indications and actual results

ESWL of gallbladder stones and subsequent adjuvant oral dissolution is a safe and comfortable therapy of symptomatic cholecystolithiasis. This method is limited on highly selected 10 to 15% of patients. Recent experience indicates that the ideal indication is given in patients with radiolucent solitary gallstones (diameter ≤2 cm) which are harboured in a gallbladder with unimpaired contractility. 1 year after ESWL 80% of those patients present with a stone free gallbladder. Computed tomography of gallstones may probably improve selection of patients and therapeutic success.

Gallbladder Agenesis: Report of a Preoperative Diagnosis With Magnetic Resonance Cholangiopancreatography

Agenesis of the gallbladder is an extremely rare congenital entity with shaded clinical and radiologic features, which make the preoperative diagnosis really challenging. Here, we report a case of a 52-year-old symptomatic female with biliary symptoms and contracted gallbladder at ultrasound (US). The final diagnosis was made with magnetic resonance cholangiopancreatography (MRCP) and the treatment was conservative. However, diagnosing this condition preoperatively is still challenging. However, with innovations in terms of biliary tract imaging technique, and with better knowledge of this entity, many unnecessary surgical procedures might be avoided.
