[Recommendations for end-of-life care in patients with chronic obstructive pulmonary disease].

2009

Over the last 20 years, advances in the treatment of patients with chronic obstructive pulmonary disease (COPD) have improved survival even among patients in the most advanced stages of the disease, such as those requiring domiciliary oxygen therapy.1,2 This improvement—in principle a positive development—has given rise to considerable clinical problems associated with the establishment of a therapeutic ceiling and the difficulty of determining prognosis in some of these patients. In this situation, the clinician should consider introducing palliative care, that is, care aimed at improving symptom control, communication, physical activity, and emotional support, in order to achieve the best…

Multidisciplinary intervention in the management of breathlessness in people with non-malignant chronic diseases in advanced situation

2018

Abstract This work shows a…

Fruit quality of Sicilian, commercially affirmed, and newly selected apple varieties: potential resources for genetic improvement and specialty marke…

2012

Several apple varieties are constantly selected for improved quality traits and introduced for cultivation and marketing in addition to a few traditional and affirmed varieties. Recently, specific fruit quality traits like enhanced flavor, crispness and juiciness have been particularly requested by consumers and are gradually replacing more conventional characteristics like fruit size, peel color and sweetness in the breeding programs. Furthermore, some of the newly selected varieties exhibit a relatively low level of plasticity, posing serious problems of adaptation when cultivation is proposed in a wide range of environments. In this most recent perspective, some of the old varieties or l…

Airway obstruction in relation to symptoms in chronic respiratory disease—a nationally representative population study

2000

Abstract We examined the severity of airway obstruction and the occurrence of respiratory symptoms in a large, nationally representative population sample and in a subgroup of subjects with chronic bronchitis and/or emphysema to obtain information for developing national prevention and treatment strategies for these diseases. The study population comprised of 7217 randomly selected subjects (aged 30 years and older) who participated in a comprehensive health examination survey. The ‘cases' were subjects diagnosed as having chronic bronchitis and/or emphysema. The survey methods comprised of questionnaires, interviews, physical measurements, including spirometry, and clinical examinations. I…

[Evolution over time in over perceivers of dyspnea in asthma].

2006

OBJECTIVE: In previous studies we identified a subgroup of patients whose perception of breathlessness was exaggerated during acute bronchoconstriction and who were termed “over perceivers” or “hyperperceivers.” In this study we aimed to determine whether such over perception is sporadic or stable over time. We also examined whether there is an association between over perception of dyspnea and hyperventilation syndrome. PATIENTS AND METHODS: The subjects were 22 stable asthmatics (11 men, 11 women) who had been over perceivers of dyspnea in a study 9 years earlier. After a medical history was taken, a patient performed forced spirometry and a severity classification was made according to t…

[Perception of dyspnea and treatment adherence in asthmatic patients].

2008

OBJECTIVE: The majority of studies show that treatment adherence in chronic diseases such as asthma does not exceed 50%. Although the reasons may vary, it is clear that lack of treatment adherence is a determining factor in poor disease control. An association has also been observed between lack of perception of dyspnea and difficult-to-control asthma and with the occurrence of fatal or near-fatal asthma attacks. In this study we therefore attempted to demonstrate that one of the reasons that asthmatic patients do not adhere to treatment is a failure to perceive dyspnea associated with bronchial obstruction. PATIENTS AND METHODS: We analyzed 2 groups of patients with moderate persistent ast…

Pathophysiological Factors in the Relationship between Chronological Age and Calculated Lung Age as Detected in a Screening Setting in Community-Dwel…

2016

Aim To explore the relationship between pathophysiological factors and premature lung aging in a cohort of community-dwelling subjects in a health-screening setting. Methods 16,107 pharmacy customers in Germany (5954 males, 10,153 females; mean age 59.7 years) participated in a lung function screening project by providing demographic data, including smoking status and known airway conditions and performing spirometry with a Vitalograph, a spirometry screening device. Lung age was calculated from the spirometric findings, and the difference between chronological age and calculated lung age was analyzed in its relationship to the demographic data in general linear models. Results In the overa…

A 15-Year-Old Boy with Anterior Chest Pain, Progressive Dyspnea, and Subcutaneous Emphysema of the Neck

2009

We describe the case of an adolescent who was admitted to the hospital because of sudden occurrence of chest pain, dyspnea and subcutaneous emphysema. On admission, physical examination revealed subcutaneous crepitations in the superior part of the rib cage, and auscultation of the chest showed widespread wheezing. The radiological assessment confirmed the diagnosis of pneumomediastinum and pneumothorax. A follow-up CT scan performed one week after the admission showed almost complete resolution of the radiological alterations. At the following visits, the patient was asymptomatic, but reported to have suffered from frequent episodes of rhinorrea, sneezing, nasal blockage, and sometimes, ch…

Molecular phylogeny, biogeographic history, and evolution of cave-dwelling taxa in the European harvestman genusIschyropsalis(Opiliones: Dyspnoi)

2015

Abstract We estimated a multigenic molecular phylogeny and reconstructed biogeographic history for the European harvestman genus Ischyropsalis C.L. Koch 1839 (Dyspnoi). To reconstruct historical biogeographic patterns we conducted an algorithmic VIP analysis which revealed patterns consistent with a vicariance-dominated history. The existing morphology-based systematic framework for Ischyropsalis is mostly inconsistent with molecular phylogenetic results, and a new informal system is established that recognizes three main clades and several sub-clades. Species-level analyses revealed two non-monophyletic species (I. pyrenaea Simon 1872 and I. luteipes Simon 1872); subspecies of I. pyrenaea …
