Führt hochdosiertes Dipyridamol in der sekundären Schlaganfallprävention zu kardialen Ereignissen?

In einer post-hoc Analyse der European Stroke Prevention Study 2 (ESPS2) wurde untersucht, ob die Gabe von Dipyridamol als Thrombozytenfunktionshemmer bei Patienten mit TIA oder Schlaganfallen das Risiko kardialer Ereignisse erhoht. ESPS2 war eine Sekundarpraventionsstudie an 6602 Patienten nach TIA oder ischamischem Insult, die in vier Armen Acetylsalicylsaure (ASS, 2×25 mg), retardiertes Dipyridamol (DP, 2×200 mg), die Kombination aus ASS plus DP oder Placebo verglich. Es ergaben sich keine Anhaltspunkte dafur, dass DP zu vermehrten kardialen Ereignissen (Angina pectoris, Myokardinfarkt, Tod) fuhrt. In westlichen Industrienationen ist der Schlaganfall die dritthaufigste Todesursache und die haufigste Ursache fur erworbene Behinderungen uberhaupt. Optimale Behandlungsbedingungen bestehen nur dann, wenn die Fruhsymptome des Schlaganfalls rechtzeitig erkannt werden und die Behandlung innerhalb von 3 h nach Einsetzen der Erstsymptome eingeleitet wird. Eine „Rettungskette des Schlaganfalls” muss deshalb Konzepte zur Fruherkennung, eine rasche prahospitale Versorgung gefolgt von effizienter Diagnostik und eine unmittelbar einsetzende Akuttherapie sinnvoll integrieren. What Is the Role of Minimally Invasive Mitral Valve Surgery in High-Risk Patients? A Meta-Analysis of Observational Studies

Background. Minimally invasive valve surgery is related to certain better postoperative outcomes. We aimed to assess the role of minimally invasive mitral valve surgery in high-risk patients. Methods. A systematic literature review identified eight studies of which seven fulfilled criteria for metaanalysis. Outcomes for a total of 1,254 patients (731 were conventional standard sternotomy and 523 were minimally invasive mitral valve surgery) were submitted to meta-analysis using random effects modeling. Although most patients having sustained a stroke or hip fracture (PSHF) attend a period of therapy and rehabilitation program, it usually only last a few months and they may not continue an exercise program when the rehabilitation treatment has ended. Many Authors suggests that PSHF can improve or maintain the rehabilitation outcomes through an exercise program. Analgesic hypnotic treatment in a post-stroke patient

In recent years, hypnotic suggestions have been used in several clinical conditions. This treatment is often used for anxiety treatment, somatization, and post-traumatic stress disorder. Hypnotic analgesia is one of the most clinically useful phenomena of hypnosis. The article describes the case of a patient who underwent hypnotic treatments for hypersensitivity and chronic pain. Results showed an improvement of pain control and a decrease of pain hypersensibility. In addition, during rehabilitative treatments, the patient reported a high level of compliance with the multidisciplinary team. These findings suggest that hypnosis could be a useful treatment for post-stroke pain management.
