Viene descritto il caso di un paziente divenuto acalasico dopo intervento di laringectomia totale allo scopo di comprendere i meccanismi fisiopatologici che possono aver determinato l’insorgenza di tale complicanza.

Abstract Background Breast cancer is the most frequent malignant tumour to metastasize into the gastrointestinal tract in female and is second only to malignant melanoma. Nevertheless gastrointestinal metastases arising from breast cancer are quite rare. The upper gastrointestinal tract is more frequently involved and lobular infiltrating carcinoma has a greater predilection compared to the ductal type. Malignant colorectal obstruction. Palliative endoscopic treatment with self-expanding metallic stents

The Authors report their experience in the palliative management of malignant colorectal obstruction. Between June 2000 and April 2003 sixteen patients (11 men and 5 women, age range 63-82) were found suffering from malignant colorectal obstruction. In these patients it was not possible to perform definitive surgical treatment because of the old age, advanced neoplastic disease and poor health condition, colostomy was the only therapeutic option. Endoscopic self-expanding stents placement was performed as a definitive palliative method to treat colorectal obstructions caused by advanced and unresectable tumours. Achalasia and carcinoma of the esophagus

Different figures regarding the relationship between achalasia and carcinoma of the esophagus can be found in the literature. Often achalasia is considered as a risk factor. The author analyzing the data collected from the Shanghai Chest Hospital (S.C.H.) and the Pathophysiology Department of the University of Palermo, both referring centers for esophageal diseases conclude that carcinoma of the esophagus in achalasic patients is just a coincidence and no relationship can be found between the two diseases.

Un raro caso di Occlusione Intestinale:Fitobezoar

Gli Autori riportano il caso di una paziente di 62 anni affetta da fitobezoar gastrico, sottoposto a frammentazione e asportazione per via endoscopica. A distanza di 36 ore circa dall'esecuzione di tale metodica, la paziente ha manifestato l'insorgenza di una sindrome occlusiva. L'esame TC addome evidenziava il digiuno e le prime anse ileali sovradistese dopo somministrazione di mdc per os. Nel lume di un'ansa ileale, a valle, si evidenziava la presenza di una formazione disomogenea, del diametro 2,5x4 cm nel cui contesto si rilevano piccole bolle di gas, da riferire in 1° ipotesi a bezoar. The aim of the study was to report our experience with the use of radiology in functional disorders of the cricopharyngeal muscle and their surgical therapy using digital cineradiology. Five-hundred and seventy dysphagic patients underwent dynamic study of the oral and pharyngeal phases of swallowing (Videofluoroscopic Swallowing Study, VFSS). A motor disorder of the cricopharyngeal muscle was diagnosed by videofluorography in 19 patients: the disorder was mild in 8, moderate in 7 and severe in 4. The routine use of intraoperative cholangiography during laparoscopic cholecystectomy remains controversial. Mirizzi was the first to recommend the use of intraoperative cholangiography in 1931 based on the high incidence of unsuspected common bile duct stones. The use of intraoperative cholangiography before common bile duct exploration reduced the incidence of unnecessary common bile duct explorations from 66% to less than 5%. With the introduction of laparoscopic cholecystectomy, an increase of incidence of bile duct injury two to four times that seen in open cholecystectomy was witnessed. RHEUMATIC PATHOLOGIES IN SUBJECTS WITH INFLAMMATORY BOWEL DISEASE

Inflammatory Bowel Disease (IBD) are commonly associated with rheumatic diseases of the joints. Sixty-five patients are selected during a 5-year study period. All Patients were analyzed with clinical, laboratory and radiographic exams. 38 patients had ulcerative colitis (mean age 42.1 years range:19-75) and 27 patients had Crohn's disease (CD) (mean age 37.2 years, range: 17-64 years). Arthritis occurred in 11 patients (17%): 7 with UC (18.4%) and 4 CD (14.8%). The mean age of patients with arthritis was 35 and mean disease duration of pain and limitation symptoms was 28 months.
