Could a variant structural form of thymidylate synthase gene of metastatic colorectal cancer patients be related with the poor response to 5 Fu treatment?
Background: Thymidylate synthase (TS) catalyzes methylation of dUMP to dTMP and is the target of 5-fluorouracil (5-Fu). TS levels vary considerably among tumors and the response to 5-FU is influenced by the intratumoral activity of the enzyme, with high levels generally being associated with a poor response. Response to 5-FU also depends on TS structure and some researchers showed that variant structural forms of TS in tumour cell lines confer resistance to fluoropyrimidines so the aim our study is to analyze the whole coding regions of the TS gene to evaluate a possible mechanism of 5 Fu resistance. Materials and Methods: We performed the TS-DNA gene sequence in 68 colorectal cancer (CRC) …
La colecistectomia laparoscopica nel paziente uremico
The aim of this study was to evaluate the effectiveness of the laparoscopic cholecystectomy performed in two uremic patients with gallstones. From January to July 2004, 15 patients with gallstones underwent to laparoscopic cholecystectomy (LC). Two patients had chronic renal failure on haemodyalisis: a symptomatic fourty-two old woman, which had primitive hyperparathyroidism and primitive hyperaldosteronism, and asymptomatic transplant candidate fifty-five old man. No minor and major complications occurred and both patients were discharged 4 days after LC. Even in uremic patients the L.C. must be the gold standard for the treatment of gallstones both symptomatic and asymptomatic.

TRATTAMENTO ENDOSCOPICO DEI POLIPI DEL COLON-RETTO IN UN AMBULATORIO DI ENDOSCOPIA DIGESTIVA:NOSTRA ESPERIENZA
The aim of this report was to evaluate the effectiveness of the endoscopic treatment of colonic polyps to allow secondary prophylaxis in order to prevent the onset of cancer arising from adenomas. From October 2002 to January 2004 we performed 487 colonoscopies on a patient group with the following indications: screening prior to kidney transplant; screening for colorectal cancer (patients positive at faecal occult blood testing); follow-up of patients who had undergone colonic resections for colorectal cancer; patients with other diseases. Colorectal polyps were diagnosed in 15 males and 15 females, with a mean age of 63 years. All the neoplasms were resected during colonoscopy and specime…

PREOP: an international study of elderly surgical oncology patients to optimize preoperative assessment
Purpose of the study: To test the predictive value of the Groningen Frailty Indicator (GFI), Vulnerable Elders Survey 13 (VES-13), timed “get up and go” and Preoperative Assessment of Cancer in Elderly (PACE) for postoperative mortality and morbidity. Method(s): International multi-center prospective cohort study. The combination of components of PACE, timed “get up and go”, the VES-13 and the GFI are administered within two weeks prior to the surgical procedure. Study population: Patients aged 70 years undergoing elective surgery for a solid tumor under general anesthesia are included. Outcome measures: Primary endpoint is 30 day morbidity and mortality. Secondary endpoints are length of h…
Giant Complicated Hydatid Cyst Of The Liver And Occult Duodenal Adenocarcinoma: A Case Report.
We present an original article about an interesting and challenging case: a patient with abdominal discomfort, dyspepsia due to a giant hydatid cyst of the liver causing obstruction of upper digestive canal, reduction of hepatic function, hepatomegaly and hiding an occult duodenal adenocarcinoma marked by a very fast evolution.
