Speciation of organic matter in natural waters. Interaction of polyacrylates with the major cation components of seawater
The speciation of some high molecular weight polycarboxylates was studied in different ionic media. Polyacrylates here investigated (W=2.0, 5.1 and 20.0 kDa) form weak species with alkali metal cations (K=102 mol l1, t=25 jC, I=0 mol l1) and quite stable complexes with alkaline earth metal cations (K>106 mol l1, t=25 jC, I=0 mol l1). Results are reported from experiments performed in a multicomponent electrolyte solution simulating the major composition of seawater (artificial seawater). Protonation constants in this medium are expressed as a polynomial function of S1/2 (S=salinity) and the sharp lowering with respect to values obtained in non-interacting aqueous tetraalkylammonium salts ca…
AMPULLECTOMIA E CARCINOMA GIGANTE DELLA PAPILLA DI VATER
The case of a giant carcinoma of Vater's papilla is reported. The clinical/diagnostic paths and therapeutic indications for ampullectomy are analysed on the basis of the good survival rate observed more than five years after transduodenal excision and in the light of the most recent literature. The authors believe that accurate, early diagnosis and histological typing is fundamental for an appropriate selection of patients when complex surgical decisions have to be taken. They consider ampullectomy to be a valid therapeutic option, certainly in the case of benign neoplasias, and duodenocephalopancreasectomy (DCP) to be the operation of choice for the treatment of ampullar and periampullar m…

[Effectiveness of contemporary injection of botulinum toxin and topical application of glyceryl trinitrate against postoperative pain after Milligan-Morgan haemorrhoidectomy].
PURPOSE: After haemorrhoidectomy the maximum resting pressure (MRP) of the anal canal is significantly increased. This increase play an important role in the making of postoperative pain. Recently, both the topical application of glyceryl trinitrate (GT) and the intrasphincter injection of botulinum toxin (Tox), resulted effective, in reducing temporary the MRP although with different mechanism of action. In this study the effectiveness and safe of contemporary injection of Tox and topical application of 300 mg/die of GT after Milligan-Morgan haemorrhoidectomy, were evaluated. MATERIALS AND METHODS: Ten patients, undergoing Milligan-Morgan haemorrhoidectomy for 3rd and 4th degree haemorrhoi…
Randomized clinical trial of botulinum toxin injection for pain relief in patients with thrombosed external haemorrhoids
Abstract Background Thrombosed external haemorrhoids are one of the most frequent anorectal emergencies. They are associated with swelling and intense pain. Internal sphincter hypertonicity plays a role in the aetiology of the pain. This study evaluated the efficacy and safety of an intrasphincteric injection of botulinum toxin for pain relief in patients with thrombosed external haemorrhoids. Methods Thirty patients with thrombosed external haemorrhoids who refused surgical operation were randomized into two groups. Patients received an intrasphincteric injection of either 0·6 ml saline or 0·6 ml of a solution containing 30 units botulinum toxin. Anorectal manometry was performed before tr…

Modelling of natural and synthetic polyelectrolyte interactions in natural waters.
In this paper SIT and Pitzer models are used for the first time to describe the interactions of natural and synthetic polyelectrolytes in natural waters. Measurements were made potentiometrically at 25 °C in single electrolyte media, such as Et4NI and NaCl (for fulvic acid 0.1 < I /mol L− 1 < 0.75), and in a multi-component medium simulating the composition of natural waters at a wide range of salinities (for fulvic and alginic acids: 5 < S < 45) with particular reference to sea water [Synthetic Sea Water for Equilibrium studies, SSWE]. In order to simplify calculations, SSWE was considered to be a “single salt” BA, with cation B and anion A representing all the major cations (Na+, K+, Mg2+…

Botulinum toxin vs. topical glyceryl trinitrate ointment for pain control in patients undergoing hemorrhoidectomy: a randomized trial.
PURPOSE: The maximum resting pressure in the anal canal is greatly raised after hemorrhoidectomy. This increase is likely to be the cause of postoperative pain, which is still the most troublesome early problem after hemorrhoidectomy. This study was designed to compare, after hemorrhoidectomy, the effects of intrasphincter injection of botulinum toxin vs. application of glyceryl trinitrate ointment in improving wound heating and reducing postoperative pain at rest or during defecation. METHODS: Thirty patients with hemorrhoids of third and fourth degree were included in the study and randomized in two groups. Anorectal manometry was performed preoperatively and 5 and 40 days after hemorrhoi…

Quality of life in elderly patients after inguinal hernioplasty
Background and aim Inguinal hernia is the most frequent abdominal wall her- nias in elderly population. Surgical treatment must be aware of the cardiovascular and respiratory disease these patients are often affected by. Inguinal hernia is responsi- ble to impair the quality of life (QoL). Aim of this study was to evaluate the QoL through the Short-Form (SF)-36 questionnaire in a cohort of elderly patients undergoing inguinal hernioplasty. Materials and methods Fifteen male patients of age ≥75 years affected by sympto- matic unilateral uncomplicated inguinal hernia were included in this study. All patients undergoing inguinal hernioplasty in local anaesthesia according to the Liech- tenstei…
Production of VEGF and b-FGF in the drainage fluid from patients undergoing incisional hernia repair
Wound healing is a complex process involving interaction between different cell types, such as growth factors. Among these, vascular endothelial growth factors (VEGF) and basic fibroblast growth factors (b-FGF) are the most important. The aim of this study was to assess the production of VEGF and b-FGF in wound drainage fluid from patients undergoing incisional abdominal hernia repair. Ten female patients with abdominal midline incisional hernia undergoing surgical repair were included in this study. In all cases a closed suction drain was placed in the wound below the fascia and removed on postoperative day 4. Wound fluid was collected on the I, II, III and IV day and its amount at each ti…

LONG-TERM MANOMETRIC STUDY OF ANAL SPHINCTER FUNCTION AFTER HEMORRHOIDECTOMY
BACKGROUND AND AIM: Data on maximum resting pressure (MRP) and maximum squeeze pressure (MSP) changes after hemorrhoidectomy are not univocal and follow-up of patients undergoing surgery is mostly short-lived. The aim of this study was to prospectively examine during 1-year follow-up the long-term manometric results of MRP, MSP, and ultra slow wave activity (USWA) within a set of patients undergoing Milligan-Morgan hemorrhoidectomy as compared to healthy controls. MATERIALS AND METHODS: Twenty patients with hemorrhoids of third and fourth degree were enrolled and anorectal manometry was performed preoperatively, on the 5th day, and after 1, 6, and 12 months after surgery. RESULTS: On the 5t…

Production of cytokines at the operation site
BACKGROUND AND AIM: Cytokines are part of a family of molecules involved in the initiation, control and termination of the events that occurs in wound healing process. Aim of this study was to evaluate the production of some cytokines [interleukin (IL)-6, IL-10, IL-1alpha, IL-1ra, interferon (IFN)-gamma] in the drainage wound fluid from patients undergoing incisional hernia repair. METHODS: Ten female patients with abdominal midline incisional hernia undergoing to surgical repair were included in this study. In all cases a closed suction drain was placed in the wound below the fascia and it was removed on the 4th postoperative day. Wound fluid was collected on the 1st, 2nd, 3rd and 4th day …
