SALIVARY PROTEOME ANALYSIS BY 2D-PAGE AND SELDI-TOF/MS TO SEARCH POTENTIAL BIOMARKERS OF ORAL SQUAMOUS CELL CARCINOMA
Aim. Oral squamous cell carcinoma (OSCC) is the most common malignant tumor of the oral cavity and is a neoplasia characterized by a poor prognosis. In fact, the estimated survival rate is 44% after 5 years from the diagnosis. The prognosis of the disease is significantly related to the stage in which the disease is diagnosed. Moreover the therapies are not very efficient and are so invasive, disfiguring and debilitating that in the survivors the quality of life is poor. Nowadays there is not a reliable and non-invasive method for early diagnosis of oral squamous cell carcinoma and the simple visual examination of the oral cavity is characterized by a low sensitivity and specificity, also b…
Oral squamous cell carcinoma as a complication of chronic graft-versus-host disease: a case report
Objectives. Chronic Graft-Versus-Host Disease (cGVHD) is an immunoregulatory disorder which occurs after allogeneic hematopoietic-cell transplantation (HCT) and often shares features of autoimmunity and immunodeficiency. GVHD related oral manifestations include severe oral pain, xerostemia, ulcerative lesions, and mucositis. Furthermore,a major late suggested complication of HSCT is the dramatically increased risk of secondary malignancies including oral cancer and oral cGVHD-releted inflammation may be considered a potential risk factor. Case report. A 51-year-old male patient referred in 1994 to our surgery for cGVHD oral lesions secondary to HCT for acute myelogenous leukemia developed i…
Transbuccal tablets of carbamazepine: formulation, release and absorption pattern.
Tranbsuccal drug administration is an attractive method, as it has several advantages especially with respect to peroral delivery. Here we report: i) the aptitude of carbamazepine (CBZ) to penetrate porcine buccal mucosa and reconstituted human oral (RHO) epithelium; ii) three different tablet formulations for transbuccal administration; iii) the drug release rate from tablets. CBZ permeation through the buccal mucosa was investigated by using two different bi-compartmental open models: Franz cells for porcine buccal mucosa and Transwell diffusion cells system for RHO epithelium. Results, expressed as drug flux (Js) and permeability coefficients (Kp), indicated that CBZ well penetrates the …

Uncommon diagnosis of Kaposi’s Sarcoma in a HIV-negative adult patient
Kaposi’s Sarcoma (KS) has been described as a multifocal malignant neoplasm, originating from vascular endothelial cells. Four clinical types has been recognized: classic, endemic, iatrogenic and epidemic1. Classic KS has been reported mainly in patients from Mediterranean area and it typically manifests as cutaneous lesions involving the lower extremities and trunk. Classic KS affects only rarely the head and neck region and few cases have been reported as primary oral classic KS. Oral lesions are localized typically in the hard palate (95%), and more rarely they affects the gingiva and the oro-pharynx. Early oral early lesions typically appear as flat pigmented macules of purple or bluish…

ENDOCANNABINOIDS CB1 AND CB2: A POSSIBLE ROLE IN ORAL SQUAMOUS CELL CARCINOMA PATHOGENESIS
Aim. Cancer of the oral cavity is the eighth most common malignancy in the world. The disease is diagnosed after a considerable delay and predict the individual progression is difficult. The improvements of the therapy did not increased the rate of survival at 5 years, that is still about 50%. The identification of the possible markers that indicates the progression of the tumor is therefore necessary. Endocannabinoids may play an important role in the process of carcinogenesis in the oral mucosa. They are able to bind and activate specific membrane receptors coupled to G proteins, cannabinoid receptors CB1 and CB2. The purpose of this study is to evaluate the expression of CB1 and CB2 rece…

Osteonecrosis of the jaw related to everolimus and bisphosphonate: a unique case report?
Osteonecrosis of the jaw (ONJ) is a rare but serious lesion of the jaw, characterized mainly by exposed necrotic bone;it is related to various drugs, usually used for treating patients with advanced malignancies. Drugs implicated in ONJ are: nitrogen-containing bisphosphonates (NBPs), denosumab, anti-angiogenic drugs (e.g bevacizumab, sunitinib, sorafenib) and the selective mammalian target of rapamycin mTOR, everolimus. Previous data regarding the combining of NBPs with antiangiogenic agents conflict with some reports (indicating a similar risk of ONJ compared with the use of NBPs alone1); other reports show significantly higher rates (18% vs 1% with NBPs alone) of the incidence of ONJ2. Th…
Salivary proteomic biomarkers of oral squamous cell carcinoma
Objectives. The aim of the present study is to investigate the presence of proteomic signatures of Oral Squamous Cell Carcinoma (OSCC) in saliva and their use as potential biomarkers for early and non-invasive diagnosis, as well as prognostication. Methods. Saliva from 45 OSCC patients and 30 healthy controls was analysed by SELDI-TOF mass spectrometry and ProteinChip® technology. Proteomic profiles were tested with differential expression analysis and fold change of protein peaks, principal component analysis, Spearman rank correlation test and hierarchical clustering in order to identify a list of peaks of interest representative of controls, N- and N+ cases. Those peaks were used in a su…

Oral mucositis: a survey on changes in the proteomic profile
Objectives. Oral mucositis is the most severe complication of anticancer therapy. It occurs in 40-85% of patients during chemotherapy and radiotherapy but also in patients who have undergone hematopoietic stem cell transplantation. The symptoms for oral mucositis are burning and severe pain oral, spontaneous bleeding, dysphagia, dysarthria and odynophagia; especially pain and burning sensation on swallowing contribute to decreased quality of life for oncologic patients and, in severe cases, may also force the patient to feed parenterally (1). Furthermore, if the mucositis is severe it can lead to partial or complete interruption of radiotherapy before completion of the treatment protocol wi…

A SIMPLE SURGICAL-MEDICAL PROTOCOL TO CARRY OUT DENTAL EXTRACTIONS IN PATIENTS IN THERAPY WITH BISPHOSPHONATES: OUR EXPERIENCE
Aim. Bisphosphonate Related OsteoNecrosis of the Jaw (BRONJ) is described as an adverse event related to amino-bisphosphonate (NBP) therapy, occurring as a result of reduced bone resorption and bone turnover. One of the most significant risk factor associated with the onset of BRONJ is tooth extraction even if not yet supported by definitive scientific evidences. The aim of this study is to propose a simple surgical/medical protocol to carry out dental extractions in patients in therapy with NBPs in order to minimize the BRONJ risk. Materials and methods. Eighty-eight patients currently or previously treated with NBPs were selected for extractions of compromised teeth. Patients were divided…
Follow-up of non-exposed ONJ related to bisphosphonate: a two-year study
Aim. Osteonecrosis of the jaw (ONJ) is a serious sideeffect of amine bisphosphonate (NBP) and/or other antiresorptive agents frequently used in the management of osteometabolic and cancer-related condition. The traditional ONJ definition exclude patients who present the non-exposed variant. However, according to recent data, staging and monitoring of ONJ should be closely linked to clinical and radiological manifestation regardless of the presence of bone exposed. The aim of this study was a descriptive analysis of a case series of patients with non-exposed ONJ (exclusively related to NBP) monitoring for two years. Methods. From 2012 to 2014, 16 patient [7 oncological (F/M=3/4; mean age=66,…
Real-time cell analysis by xCELLigence®: a new method for dynamic, quantitative measurement of adhesion and proliferation of cell lines
Objectives. In this study, we report the use of a real-time cell analysis (RTCA) test system, the xCELLigence® RTCA, as efficient tool for a fast growth kinetics analysis of cell lines. This new dynamic real-time monitoring and impedance-based assay allows for a combined measurement of cell adhesion, spreading and proliferation. Methods. We used four representative human OSCC derived cell lines, PE49, HSC2, HSC3 and PE15 cells. The measured impedance values could be correlated to characteristic cell culture behaviours. In parallel, were evaluated proliferation and cell viability of the cell lines by the 3-[4,5-dimethylthizol-2-yl]-2,5-diphenyltetrazolium bromide (MTT) assay. Results. Throug…
New perspectives in medical approach to therapy of head and neck squamous cell carcinoma
Head and neck cancer is the eighth most common cause of cancer death worldwide. Its incidence varies widely among different regions. In North America and the European Union, head and neck cancer accounts for 3% to 4% of all cancer diagnoses. Conversely, in Southeast Asia and Africa, head and neck cancer accounts for approximately 8% to 10% of all cancers1. Although the incidence of head and neck cancers has decreased slightly from 1975 to 2002 in the United States5, approximately 46 000 new cases are still expected in 2007 alone. Even if surgery and radiotherapy have remained the core therapy in squamous cell carcinoma of the head and neck (HNSCC). Radiotherapy following surgery was the sta…

Update on gingival overgrowth by cyclosporine A in renal transplants
Severe gingival overgrowth is one of the most frequent side effects in renal transplant patients associated with assumption of cyclosporine A. Several associations with age, sex, dosage, duration of therapy or interval since transplantation have been hypothesized. The introduction of alternative immunosuppressant drugs have been suggested to permit better long-term transplant outcomes and a decrease in incidence of gingival overgrowth. The aim of the present paper is to summarize current knowledge regarding aetiology, pathogenesis and management of gingival overgrowth induced by Cyclosporine A. El sobrecrecimiento gingival severo es uno de los efectos adversos más frecuentes en los paciente…
