Tidal volume and helmet: Is the never ending story coming to an end?
2021
Noninvasive ventilation (NIV) has been increasingly used in acute care setting with various indications, but its use in patients with acute hypoxemic respiratory failure (AHRF) is controversial. In this setting, reliable monitoring of Vt and unintentional leaks is of the utmost importance. The aim of this article is to describe the importance of tidal volume measurement in the context of AHRF treated with NIV.
Use of Helmet CPAP in COVID-19 – A practical review
2021
Helmet CPAP (H-CPAP) has been recommended in many guidelines as a noninvasive respiratory support during COVID-19 pandemic in many countries around the world. It has the least amount of particle dispersion and air contamination among all noninvasive devices and may mitigate the ICU bed shortage during a COVID surge as well as a decreased need for intubation/mechanical ventilation. It can be attached to many oxygen delivery sources. The MaxVenturi setup is preferred as it allows for natural humidification, low noise burden, and easy transition to HFNC during breaks and it is the recommended transport set-up. The patients can safely be proned with the helmet. It can also be used to wean the p…
Influence of different interfaces on synchrony during pressure support ventilation in a pediatric setting: a bench study
2015
BACKGROUND: In adults and children, patient-ventilator synchrony is strongly dependent on both the ventilator settings and interface used in applying positive pressure to the airway. The aim of this bench study was to determine whether different interfaces and ventilator settings may influence patient-ventilator interaction in pediatric models of normal and mixed obstructive and restrictive respiratory conditions. METHODS: A test lung, connected to a pediatric mannequin using different interfaces (endotracheal tube [ETT], face mask, and helmet), was ventilated in pressure support ventilation mode testing 2 ventilator settings (pressurization time [Timepress]50%/cycling-off flow threshold [T…
The effect of tiotropium/olodaterol versus fluticasone propionate/salmeterol on left ventricular filling and lung hyperinflation in patients with COPD
2020
This exploratory, randomised, double-blind, double-dummy, multicentre, cross-over study explored the effect of 6 weeks of treatment with tiotropium/olodaterol (T/O) versus fluticasone propionate/salmeterol (F/S) on left ventricular filling in patients with chronic obstructive pulmonary disease with functional residual capacity (FRC) >120% predicted and postbronchodilator improvement of FRC ≥7.5%. Overall, 76 patients were randomised across nine sites. Treatment with T/O or F/S increased left ventricular end-diastolic volume index from baseline (adjusted mean change: T/O: 2.317 mL/m2, F/S: 2.855 mL/m2), with no statistically significant difference between treatments. However, T/O resulted…
Lipid-containing semipermeable membrane devices (SPMDs) as concentrators of toxic chemicals in the lower Fraser River, Vancouver, British Columbia.
1998
Abstract Semipermeable membrane devices (SPMDs) were deployed in the water column and in the sediments of the Lower Fraser River (B.C., Canada) to compare the levels and the congener profiles of polychlorinated dibenzo- p -dioxins (PCDDs), dibenzofurans (PCDFs) and non- ortho -chlorinated biphenyls (N-o,o′-PCBs) against those sampled by an Infiltrex resin column water sampler. Also the relationship between what was present in the ecosystem as sampled by SPMDs and what was accumulated in the tissue of resident benthic-feeding fish was explored. Only the lipid portion of the SPMDs was prepared for analysis, due to some technical difficulties in cleaning the polyethene tubing. The calculated w…
Narrative Medicine to integrate patients’, caregivers’ and clinicians’ migraine experiences: the DRONE multicentre project
2021
Abstract Background Although migraine is widespread and disabling, stigmatisation and poor awareness of the condition still represent barriers to effective care; furthermore, research on migraine individual and social impact must be enhanced to unveil neglected issues, such as caregiving burden. The project investigated the migraine illness experience through Narrative Medicine (NM) to understand daily life, needs and personal resources of migraneurs, their caregivers and clinicians, and to provide insights for clinical practice. Methods The project involved 13 Italian headache centres and targeted migraneurs, their caregivers and migraine specialists at these centres. Written narratives, c…
Effects of Scientific Information Format on the Comprehension Self-Monitoring Processes: Question Generation
2013
The opportunity of using wave energy converters in a Mediterranean hot spot
2022
The opportunity of using wave energy converter arrays in a hot spot area of the Mediterranean sea was investigated. For this purpose, numerical simulations of wave energy converters together with a simple cost-effective analysis were carried out. The selected study area is one of the most energetic inside the Sicilian channel (Italy). The numerical simulations were performed using the SNL-SWAN (“Sandia National Laboratories – Simulating WAves Nearshore”) spectral model with an unstructured mesh. The power absorbed from thirteen Wave Energy Converter (WEC) devices was computed. In particular, Pelamis, Wave Dragon, Oyster 2, Wavestar, B-HBA, B-OF, Bref-HB, Bref-SHB, F–2HB, F–3OF, F-HBA and F-…
Functional evaluation of a “lingual ring” oral device applied on patients affected by temporo-mandibular disorders tmds: A comparative clinical trial
2021
This study aims to evaluate the functional variations of the electromyographic response and clinical symptomatology in TMD patients. This study has been performed and compared before and after the application of a therapeutic protocol based on the use of an oral device working on the proper mandibular repositioning through the proprioceptive-based lingual re-education called “Lingual Ring”. Between January to December 2016, 32 TMD patients have been recruited out of a series of 321 individuals recruited at the Neurosensorial and Motorial Surgery Department, in Palermo (Italy). All the patients underwent the Surface Electromyography (sEMG) of Masseter and Temporal muscles, with different reg…
