Laboratory testing in the emergency department: an Italian Society of Clinical Biochemistry and Clinical Molecular Biology (SIBioC) and Academy of Em…

2018

Abstract Background: The mainstay of patient-oriented laboratory testing in emergency settings entails selecting a number and the type of tests according to valid criteria of appropriateness. Since the pattern of urgent tests requesting is variable across different institutions, we designed a joined survey between the Academy of Emergency Medicine and Care (AcEMC) and the Italian Society of Clinical Biochemistry and Clinical Molecular Biology (SIBioC) for reaching tentative consensus about the most informative diagnostic tests in emergency settings. Methods: A survey, containing the most commonly ordered urgent laboratory tests and the relative clinical indications, was disseminated to eigh…

Oleic Acid-Injection in Pigs As a Model for Acute Respiratory Distress Syndrome

2018

The acute respiratory distress syndrome is a relevant intensive care disease with an incidence ranging between 2.2% and 19% of intensive care unit patients. Despite treatment advances over the last decades, ARDS patients still suffer mortality rates between 35 and 40%. There is still a need for further research to improve the outcome of patients suffering from ARDS. One problem is that no single animal model can mimic the complex pathomechanism of the acute respiratory distress syndrome, but several models exist to study different parts of it. Oleic acid injection (OAI)-induced lung injury is a well-established model for studying ventilation strategies, lung mechanics and ventilation/perfus…

The role of US examination in the management of acute abdomen.

2013

Acute abdomen is a medical emergency, in which there is sudden and severe pain in abdomen of recent onset with accompanying signs and symptoms that focus on an abdominal involvement. It can represent a wide spectrum of conditions, ranging from a benign and self-limiting disease to a surgical emergency. Nevertheless, only one quarter of patients who have previously been classified with an acute abdomen actually receive surgical treatment, so the clinical dilemma is if the patients need surgical treatment or not and, furthermore, in which cases the surgical option needs to be urgently adopted. Due to this reason a thorough and logical approach to the diagnosis of abdominal pain is necessary. …

Adnexal Torsion in Pediatric Age: Does Bolli’s Score Work? Report of Two Cases

2019

Adnexal torsion is a surgical emergency requiring early diagnosis in order to avoid demolitive surgery. Adnexal torsion’s diagnosis could be very difficult in pediatric patients because children cannot explain symptoms accurately. Furthermore reproductive organs lie high in abdomen, causing unclear examinations findings. For reducing diagnostic mistakes or delay clinical and hematological criteria could be useful. No radiological criteria (CT or MRI) should be taken in count because of the costs and the required time. By combining clinical presentation in patients with OT three useful diagnostic variables have been identified: age, duration of pain, vomiting. Presence of vomiting, short dur…

Clinical Cases on Colorectal Cancer Emergency

2019

You have to manage in the emergency room a 75-year-old man with a medical history of pulmonary embolism 4 months ago and currently treated with warfarin. He has no history of abdominal surgery. He consults for diffuse abdominal pain, cessation of flatus and stools for 4 days, and fever of 38.7 °C and chills for 24 h. He vomited once 24 h ago. He also experienced asthenia and recent weight loss of 7 kg over the past 6 months not attributable to diet or exercise. He complains of progressively aggravating alternation of diarrhea and constipation for 3 months. He finally remembers that he noticed several times in the last months the presence of red blood in the stools, about which he did not wo…

0349: Transfer of acute coronary syndrome patients in the Alps (SCA-Alpes). Data from the RESURCOR network

2016

RationaleIn France, the transfer of patients with an acute coronary syndrome (ACS) from one centre to another is usually done by SMUR, with on-board medical staff. However, SMUR is not always available. In this study from the RESURCOR network, we investigated clinical outcomes in ACS patients transferred in private ambulances without on-board medical staff.MethodIn the French Alps, the RESURCOR network arranges “SCAAlpes” transfers in private ambulances with no medical doctors on board but with non-medically trained staff who follow strict management protocols. Ambulances are equipped with an automatic external defibrillator and patients are not under heart monitoring. These transfers are f…

Speckle tracking analysis in intensive care unit: A toy or a tool?

2018

The use of conventional echocardiography in the intensive care unit (ICU) is today established to assess left and right ventricular systolic function, for preload determination and procedural guidance. Next step in ICU echocardiography could be the use of novel ultrasound techniques such as strain echocardiography to assist in the management of patients with acute coronary syndrome, heart failure, or pulmonary embolism. This review has gathered the available evidence supporting the incremental value of strain in the diagnostic workup of cardiac diseases treated in ICU.

Emergency surgery in a patient with large spontaneous spinal epidural hematoma determining excellent neurological recovery: Review of the literature

2014

Study design: Case report. Objectives: We report a case of a 75-year-old woman suffering from voluminous idiopathic spontaneous spinal epidural hematoma (SSEH) that was rapidly diagnosed and successfully treated. Methods: Clinical presentation was characterized by sudden and intense back pain that rapidly evolved into plegia of the right leg and severe paresis of the left leg. Hypoesthesia below T6 and urinary retention were also present. Magnetic resonance imaging showed a significant posterior spinal compression from T6 through L3 caused by an epidural hematoma that involved 10 metameric levels, extending for ∼20 cm, with a maximum thickness of 1.6 cm from T12 to L1. Results: Within 12 h,…
