A substantial decrease in Helicobacter pylori-associated peptic ulcer disease has been observed during the last decades. Drug-related ulcers as well as idiopathic ulcers are becoming predominant and are more refractory to treatment; however, H. pylori infection still plays an important role in ulcer bleeding and recurrence after therapy. The effect of H. pylori eradication upon functional dyspepsia symptoms has been reviewed in this article and generally confirms the results of previous meta-analyses. It is well established that cancer arises in chronically inflamed tissue, and this is particularly notable in the gastrointestinal tract. Classic examples include Helicobacter pylori-associated gastric cancer, hepatocellular carcinoma, and inflammatory bowel disease-associated colorectal cancer. Is Helicobacter pylori status relevant in the management of GORD?

Summary There is growing interest in the relationship between H. pylori infection and gastro-oesophageal reflux disease (GORD). However, this relationship is complex, as yet not fully elucidated, and probably based on a multiplicity of factors. The prevalence of H. pylori infection in patients with GORD is similar, more often lower than in matched controls. There is a negative correlation between H. pylori infection and the severity of GORD. There are many hypothetical mechanisms by which H. pylori infection may protect from the development of GORD. Duodenogastrischer Reflux nach Vagotomie und Pyloroplastik

Bei 27 Patienten mit selektiver oder trunkularer Vagotomie und Pyloroplastik wurde gepruft, ob ein duodenogastrischer Reflux vorliegt. Die Untersuchung erfolgte rontgenologisch durch Instillation von verdunntem Bariumbrei in das Duodenum und durch Nachweis von i.v. appliziertem Bromsulphalein im Magensaft. Es konnte bei 20 von 23 Patienten rontgenologisch und bei 18 von 27 Patienten durch den Bromsulphalein-Nachweis ein duodenogastrischer Reflux festgestellt werden. In Zusammenhang mit Reflux standen postoperative Beschwerden wie Vollegefuhl, Brechreiz und Oberbauchschmerzen, sowie der Nachweis morphologischer Veranderungen der Magenschleimhaut im Sinne von Gastritiden verschiedener Stadien.

Resolution of Ménétrier’s Disease after<i> Helicobacter pylori</i> Eradicating Therapy

Some cases of Ménétrier’s disease associated with <i>Helicobacter pylori</i> (HP) have recently been reported in the literature. We report here the case of a 51-year-old man with a diagnosis of Ménétrier’s disease who had previously been unsuccessfully treated with H<sub>2</sub> antagonists. A subsequent demonstration of HP infection led us to treat the patient with an eradicating therapy which prompted complete regression of clinical symptoms, resolution of the gastric endoscopic picture, and absence of HP on gastric histology.
