Skip metastases to lateral cervical lymph nodes in differentiated thyroid cancer: a systematic review.

Abstract Papillary thyroid carcinoma is a slow-growing cancer with a generally good prognosis that sometimes have an aggressive behaviour. Metastases to neck lymph nodes is the first step of the diffusion. The central neck compartment is involved most commonly. The ipsilateral lateral neck compartments are usually involved afterwards, and the involvement of the contralateral one is considered a quite rare occurrence. Rheumatoid arthritis (RA) may include rare extra-articular manifestations, and particularly cardiopulmonary involvement. We report a 83-year-old woman, with RA, who presented with both pulmonary and heart failures that were related to the RA, resulting from the presence of mediastinal and axillary lymph nodes, and pleural and pericardial effusions. This was confirmed by the dramatic improvement after a specific treatment of RA.

Ankle oedema and abdominal swelling suddenly developed in a 55-year-old woman who also had lymphadenopathy in the neck, axillae and groin. Ultrasonography demonstrated hepatosplenomegaly, ascites and pleural effusions. Histological examination of some lymph-nodes from the axilla and groin revealed angioimmunoblastic lymphadenopathy (low-malignant peripheral T cell lymphoma). Bone-marrow biopsy was undertaken because of a normocytic anaemia (haemoglobin 4.9 g/dl) requiring blood transfusion, thrombocytopenia (5000/microliters) and monoclonal IgG gammopathy. Laparoscopic sentinel lymph node mapping and excision of the rectum using a radioactive tracer — a prospective experimental study

Introduction: Patients with a low risk T1 rectal carcinoma can undergo the therapy of a local excision. In these patients the lymph node status remains unknown. There is a potential risk of up to 7% for nodal metastasis, in high risk cases it can rise up to 35%. To investigate the possibility of using the Sentinel lymph node (SLN) concept, which is anchored in the therapy of malignant melanoma and breast cancer, an experimental study on pigs was undertaken. The main goal of preoperative chemotherapy is to reduce the size of the tumor and allow conservative treatment. Neoadjuvant treatment can affect axillary status with a downstaging in one third of the cases. For these patients, the benefit of axillary node dissection is questioned and the sentinel node biopsy (SLNB) seems to be a relevant option. However, the timing of performing SLNB is still debated especially for clinical negative patients with negative axillary ultrasound before preoperative chemotherapy. For axillary positive nodes proved by biopsy/cytology before preoperative chemotherapy, SLNB can be an option if there is a good clinical and radiological response.

Sonography and computed tomography are used in staging of lymph nodes of patients with head and neck cancer. The accuracy of sonography (90%) and computed tomography (85%) is comparable or better than the palpatory accuracy (85%). The better delineation of reactive swollen cervical nodes leads to a higher sensitivity of sonography (90%) and computed tomography (84%) versus palpation (74%), but a lower specifity (palpation 94%, sonography 90%, computed tomography 86%). A literature survey shows that sonography, computed tomography and magnetic resonance imaging of cervical lymph nodes are comparable good methods.

Abstract Mutations within the FMS-like tyrosine kinase 3 (FLT3) gene on chromosome 13q12 have been detected in up to 35% of acute myeloid leukemia (AML) patients and represent one of the most frequently identified genetic alterations in AML. Over the last years, FLT3 has emerged as a promising molecular target in therapy of AML.
