Metacognitive group training for forensic and dangerous non-forensic patients with schizophrenia: A randomised controlled feasibility trial

2014

BACKGROUND: In schizophrenia, the presence of certain cognitive biases has been established. Informed by this, metacognitive training (MCT) has been developed for schizophrenia. There is increasing evidence of its effectiveness with some patients, but its applicability to dangerous patients has not yet been demonstrated. AIMS: Our aim was to test the feasibility of a randomised controlled trial (RCT) of MCT for patients in a high-security hospital setting. METHODS: Twenty of 33 eligible and selected male in-patients with schizophrenia and a history of violence were randomised pairwise to eight sessions of MCT or treatment as usual. Symptom severity and reasoning, according to the jumping to…

Abstract Purpose Insufficient and poor sleep patterns are common among adolescents worldwide. Up to now, the evidence on adolescent sleep has been mostly informed by country-specific studies that used different measures and age groups, making direct comparisons difficult. Cross-national data on adolescent sleep that could inform nations and international discussions are lacking. We examined the sleep patterns of adolescents across 24 countries and by gender, age, and affluence groups. Methods We obtained sleep data on 165,793 adolescents (mean age 13.5 years; 50.5% girls) in 24 European and North American countries from the recent cross-sectional Health Behaviour in School-aged Children sur…

Unmet needs in the management of schizophrenia

2014

Studies on unmet needs during the last decades have played a significant role in the development and dissemination of evidence-based community practices for persistent schizophrenia and other severe mental disorders. This review has thoroughly considered several blocks of unmet needs, which are frequently related to schizophrenic disorders. Those related to health have been the first block to be considered, in which authors have examined the frequent complications and comorbidities found in schizophrenia, such as substance abuse and dual diagnosis. A second block has been devoted to psychosocial and economic needs, especially within the field of recovery of the persistently mentally ill. Wi…

Belastung und psychosoziale Betreuungsbedürftigkeit von Glioblastompatienten

2011

Glioblastoma patients should be provided with a professional health care system that helps reduce their psychosocial burden. The aim of this study was to identify patients in need of psychosocial intervention. In addition, it was examined whether physicians' assessments adequately address the burden patients are under and their need for intervention. During their visit to one of two neurosurgery outpatient departments, n = 49 glioblastoma patients filled out the short version of the Hornheider questionnaire (HFK). Consulting physicians also rated their patients' burdens in a specially adapted version of the questionnaire (HFK-F). The results of the psychometric evaluation with both instrume…

International Perspectives on the Practical Application of Violence Risk Assessment: A Global Survey of 44 Countries.

2014

Mental health professionals are routinely called upon to assess the risk of violence presented by their patients. Prior surveys of risk assessment methods have been largely circumscribed to individual countries and have not compared the practices of different professional disciplines. Therefore, a Web-based survey was developed to examine methods of violence risk assessment across six continents, and to compare the perceived utility of these methods by psychologists, psychiatrists, and nurses. The survey was translated into nine languages and distributed to members of 59 national and international organizations. Surveys were completed by 2135 respondents from 44 countries. Respondents in al…

Consumer Satisfaction with Outpatient Mental Health Care: Impact of an Organizational Change in Mental Health Center Function from Hospital District …

1996

In Finland the administrative responsibility for outpatient psychiatric care has in part been reorganized from specialized psychiatric treatment organizations to health centers. This change has been encouraged by economic pressures, but data about its impact on the quality of treatment and patient satisfaction are limited. We carried out a follow-up study by using the UKU Consumer Satisfaction Rating Scale (UKU-CONSAT) and qualitative interviews to estimate consumer satisfaction with the psychiatric outpatient care just before and 1 year after such a change. The results suggest that dissatisfaction may emerge if too little attention is paid to the longevity of the treatment relationships an…

Sudden Complex Hallucinations in a 14-Year-Old Girl: Schizophrenia Spectrum Disorders Versus Dissociative Disorders-The Influence of Early Life Exper…

2020

A 14-year-old girl is taken to the pediatric emergency department (ED) by her parents because of sudden complex hallucinations. She shows moderate intellectual disability without a peculiar phenotype already evaluated by neuropediatrics (normal comparative genomic hybridization array and brain magnetic resonance imaging) and most likely related to affective deprivation at the orphanage. Adopted at 20 months from China, her parents reported that on arrival she had scars, allegedly from being tied to a chair for a long time, and that although she was able to stand, she could not walk. At 22 months, she had experienced several episodes of loss of consciousness with rigidity that were attributed…

Abstract Background The long-term effectiveness of antipsychotic maintenance treatment after first-episode psychosis (FEP) is contested. In this real-world observational study, we examined how cumulative exposure to antipsychotics within the first 5 years from FEP was associated with the 19-year outcome. Methods Finnish national registers were used to detect all patients who were hospitalized due to non-affective psychosis in the mid-1990s, and who were treatment naïve prior to the inclusion period (N = 1318). Generalized linear models with logit link function were used to estimate how cumulative exposure to antipsychotics within the first 5 years from onset was associated with mortality, w…
