Myotomy of cricopharungeal muscle in the therapy of dyskinesia of upper esophageal sphincter: personal experience
2012
Dyskinesia of the esophagus includes the entire esophagus from the upper esophageal sphincter to the lower esophageal sphinc - ter together with a wide spectrum of physi - opathological and clinical variations. Related to functional diseases such as Dyskinesia of UES (Upper Esophageal Sphincter), myo- tomy is an excellent procedure for patients with cervical dysphagia. In our experience, the surgical treatment is the only procedure to treat UES dyskinesia. For a successful in - tervention is mandatory: a pharyngeal pump efficiency, hypertension of UES and conservation of esophageal peristalsis in the absence of GERD (Gatroesophageal Reflux Disease). From January 2006 to December 2011, the A…

Effect of Fibrin Sealant on Seroma Formation Post-Incisional Hernia Repair
2020
Background: Development of seroma after incisional hernia repair is a common complication. Our study aims to compare the outcomes of sublay incisional hernia repairs with or without spray instillation of fibrin glue. Methods: We enrolled fifty patients undergoing incisional hernia repair. In all patients a suction drain was placed in the subcutaneous space. In one group (FG) 4 ml fibrin glue was instilled in the subcutaneous space. In the control group (C) patients did not receive any prevention measure. Wounds fluids were collected on post-operative day 1 (POD) and once daily until drain removal. All patients were followed up by ultrasound on POD 15, 20, 40, 60. Results: Drain fluid produc…
Protection of Mice from Acute Graft-versus-Host Disease Requires CD28 Co-stimulation on Donor CD4+ Foxp3+ Regulatory T Cells
2017
Acute graft-versus-host disease (aGvHD) is a major cause of morbidity and mortality after allogeneic hematopoietic stem cell plus T cell transplantation (allo-HSCT). In this study, we investigated the requirement for CD28 co-stimulation of donor CD4\(^{+}\) conventional (CD4\(^{+}\)CD25\(^{-}\)Foxp3\(^{-}\), Tconv) and regulatory (CD4\(^{+}\)CD25\(^{+}\)Foxp3\(^{+}\), Treg) T cells in aGvHD using tamoxifen-inducible CD28 knockout (iCD28KO) or wild-type (wt) littermates as donors of CD4\(^{+}\) Tconv and Treg. In the highly inflammatory C57BL/6 into BALB/c allo-HSCT transplantation model, CD28 depletion on donor CD4\(^{+}\) Tconv reduced clinical signs of aGvHD, but did not significantly pro…
Identity vicissitudes in work organizations
2014
Today’s pressing scientific and technological changes, while having an impact on the organizational life of our post-industrial world, are producing drastic transformations within organizations, creating in workers new feelings of anxiety and uncertainty. The features of the present state of crises - instability, uncertainty, weakening of family bonds – define a peculiar social and psychological uneasiness, proper of our time, as a consequence of the technological business culture prevailing today, which is destabilizing the institutional role of organizations, namely fixing the various forms of personal identity. This contribution offers, also by presenting a training experience in a busin…
Control Of Organ Transplant-Associated Graft-versus-Host Disease By Activated Host Lymphocyte Infusions
2004
Background Prolonged persistence of donor-derived T cells after organ transplantation has been proposed to improve long-term allograft survival. However, surviving transplant-derived T cells are also able to mediate devastating graft-versus-host disease (GvHD). Currently, GvHD after organ transplantation is usually refractory to conventional therapy and the disease outcome fatal. Methods Graft-reactive host T cells were generated ex vivo from a patient suffering from a severe and refractory liver-transplant-associated GvHD. To control GvHD, activated alloreactive host T cells were repetitively retransferred into the patient (activated host lymphocyte infusion [aHLI]). Results Adoptive trans…
Abnormal Nailfold Capillaries in Patients after Hand Transplantation
2020
Background: The development of graft vasculopathy may play a role in the long-term deterioration of hand grafts. The aim of study was to examine the patterns of the nailfold capillaries in hand transplant recipients. Methods: the study was performed on six patients who received hand transplantation. To normalize for the effect of immunosuppression an age- and sex-matched group of 12 patients with active kidney transplant was selected. As an additional control group, 12 healthy volunteers were recruited. Nailfold videocapillaroscopy was performed in all participants. Additionally, serum concentrations of vascular endothelial growth factor (VEGF) were measured. Results: Videocapillaroscopic e…
Effect of suturing the femoral portion of a four-strand graft during an ACL reconstruction
2014
Purpose A suture passed along the part of the graft that will be inserted into the femoral tunnel is widely used by surgeons, because it could prevent the graft sliding on the femoral fixation device during pulling from the tibial side. The aim of this study was to evaluate the biomechanical effects of suturing the intratunnel femoral part of the graft during an anterior cruciate ligament (ACL) reconstruction. Methods Bovine digital extensor tendons and tibias were harvested from 20 fresh-frozen mature bovine knees ranging in age from 18 to 24 months. Quadruple-strand bovine tendons were passed through the tibial tunnel and secured distally with a bioabsorbable interference screw. In one ha…
Diagnostic Performance of an Artificial Intelligence System in Breast Ultrasound.
2021
Objectives We study the performance of an artificial intelligence (AI) program designed to assist radiologists in the diagnosis of breast cancer, relative to measures obtained from conventional readings by radiologists. Methods A total of 10 radiologists read a curated, anonymized group of 299 breast ultrasound images that contained at least one suspicious lesion and for which a final diagnosis was independently determined. Separately, the AI program was initialized by a lead radiologist and the computed results compared against those of the radiologists. Results The AI program's diagnoses of breast lesions had concordance with the 10 radiologists' readings across a number of BI-RADS descri…
