Muscle strength and range of movement deficits 1 year after hip resurfacing surgery using posterior approach.

2009

The aim was to evaluate the effect of the posterior surgical approach on hip muscle strength and range of movement (ROM).Maximal isometric external and internal rotator strength of the hip muscles and bilateral leg extension strength were measured by dynamometers. Active (AROM) and passive (PROM) range of motion of the hip were measured by goniometers in 30 patients before and 3, 6 and 12 months after hip resurfacing. Postoperatively the patients were advised to return to their usual physical activities.Prior to the surgery internal rotation strength was on the same level on both sides, whereas external rotation strength of the operated hip was 26% (p0.001) lower compared with the unoperate…

Structural validity of the Finnish Manchester-Oxford Foot Questionnaire (MOXFQ) using the Rasch model.

2019

Background: The 16-item patient-reported Manchester-Oxford Foot Questionnaire (MOXFQ) with subscales of pain, social interactions, and walking/standing has been claimed for strongest scientific evidence in measuring foot and ankle complaints. This study tests the validity of the Finnish MOXFQ for orthopaedic foot and ankle population using the Rasch analysis. Methods: We translated the MOXFQ into Finnish and used that translation in our study. MOXFQ scores were obtained from 183 patients. Response category distribution, item fit, coverage, targeting, item dependency, ability to measure latent trait (unidimensionality), internal consistency (Cronbach's alpha), and person separation index (PS…

External rotation strength deficit after hip resurfacing surgery.

2008

The aim was to assess the effect of resurfacing hip arthroplasty on hip muscle function, when, in the posterior approach, the piriformis tendon and outer rotators of the hip joint were divided. The unoperated side was used as a control.Thirty-three consecutive patients (mean age 55 years, 73% males) were operated using the posterior approach. Main outcome measures were isometric muscle strength of external and internal rotators and extensors of the hip and hip pain.Hip external rotation strength (p0.001) and leg extension strength (p0.001) were significantly lower on the operated side compared with the un-operated side prior to the surgery. On the operated side external rotation strength de…

The ceiling effects of patient reported outcome measures for total knee arthroplasty

2020

Abstract Background Patient reported outcome measurements (PROMs) that exhibit a substantial ceiling effect show clustering of participant's scores towards the upper limit of a scale and consequently have low discriminatory power among high end scores. This study aimed to compare ceiling effects at 1 and 2 years postoperatively across commonly usedPROMs for TKA. Hypothesis We hypothesized, that the analyzed PROMs differ substantially in regards to their ceiling effect. Patients and methods Patients that underwent a primary unilateral TKA and completed pre-operative and post-operative questionnaires were included in the analysis. Participants completed the KOOS, KOOS-12, KOOS-JR, KOOS-PS, WO…

Association between lower extremity muscle strength and acute ankle injury in youth team-sports athletes

2021

Abstract Objectives To investigate lower extremity muscle strength as risk factor for an acute ankle injury in youth athletes. Design Cohort study. Setting Basketball and floorball clubs. Participants 188 youth (≤21) male and 174 female athletes. Main outcome measures 1RM leg press, maximal concentric isokinetic quadriceps and hamstrings as well as maximal isometric hip abductor strength were measured and athletes were followed for an acute ankle injury up to three years. Cox regression models were used in statistical analyses. Results In males, greater 1RM leg press and maximal quadriceps strength increased the risk of any type of acute ankle injury (Hazard ratio [HR] for 1 SD increase, 1.…

Fragility Fractures of the Pelvis.

2017

Fragility fractures of the pelvic ring (FFP) show an increasing frequency. Trauma mechanism, fracture morphology and degree of instability are different from those in high-energy pelvic ring lesions. Little is known about the optimal treatment strategy. A new comprehensive classification system with four categories of increasing instability is presented. It is connected with recommendations for type and invasiveness of treatment. FFP Type I are anterior instabilities only and can be treated conservatively. FFP Type II are non-displaced posterior lesions which can be treated conservatively or with percutaneous fixation. FFP Type III lesions are unilateral displaced posterior lesions which re…

Acute injuries in Finnish junior floorball league players

2018

Abstract Objectives To investigate the incidence and characteristics of acute time-loss injuries in Finnish junior floorball league players. Design Prospective cohort study with 3-year follow-up. Methods One hundred and eighty-six female and male players (mean age 16.6 ± 1.4) took part in the follow-up study (2011–2014). The training hours and games were recorded on a team diary. Floorball related acute injuries were registered and verified by a research physician. The injury incidence was expressed as the number of injuries per 1000 h of exposure. Incidence rate was calculated separately for games and practices, and for males and females. Results One hundred and forty-four acute time-loss …

Effect of total knee replacement surgery and postoperative 12 month home exercise program on gait parameters

2017

Abstract Objective To evaluate the effects of surgery and a postoperative progressive home exercise program on gait parameters among individuals operated with total knee arthroplasty. Design Single blinded randomized controlled trial. Subjects 108 patients (84 females, 24 males, mean age 69 years). Interventions Patients were equally randomized into an exercise group (EG) and control group (CG). The 12-months progressive home exercise program starting two months postoperatively was compared to usual care. Methods Gait analysis was performed using the Gaitrite electronic walkway system. In addition, knee extension and flexion strength were measured by a dynamometer preoperatively, and pain o…

Modular megaprosthesis reconstruction for oncological and non-oncological resection of the elbow joint

2016

Abstract Background Reconstruction of large bone defects around the elbow joint is surgically demanding due to sparse soft tissue coverage, complex biomechanics and the close proximity to neurovascular structures. Modular megaprostheses are established reconstruction tools for the elbow, but only small case series have been reported in the literature. Methods Thirty-six patients who underwent reconstruction of the elbow joint with a modular megaprosthesis were reviewed retrospectively. In 31 patients (86.1%), elbow replacement was performed after resection of a bone tumour, whereas five non-oncological patients (13.9%) underwent surgery because of a previous failed elbow reconstruction. Fun…

Global trends of hand and wrist trauma: A systematic analysis of fracture and digit amputation using the Global Burden of Disease 2017 Study

2020

BackgroundAs global rates of mortality decrease, rates of non-fatal injury have increased, particularly in low Socio-demographic Index (SDI) nations. We hypothesised this global pattern of non-fatal injury would be demonstrated in regard to bony hand and wrist trauma over the 27-year study period.MethodsThe Global Burden of Diseases, Injuries, and Risk Factors Study 2017 was used to estimate prevalence, age-standardised incidence and years lived with disability for hand trauma in 195 countries from 1990 to 2017. Individual injuries included hand and wrist fractures, thumb amputations and non-thumb digit amputations.ResultsThe global incidence of hand trauma has only modestly decreased since…
