Median-to-Ulnar Nerve Communication in Carpal Tunnel Syndrome: An Electrophysiological Study

2021

The median-to-ulnar communicating branch (MUC) is an asymptomatic variant of the upper limb innervation that can lead to interpretation errors in routine nerve conduction studies. The diagnosis of carpal tunnel syndrome (CTS) or ulnar nerve lesions can be complicated by the presence of MUC. In this study, we describe electrophysiological features of MUC in CTS patients presenting to our clinic. We enrolled MUB cases from consecutive CTS patients referred to our laboratory between the years 2014 and 2019. MUC was present in 53 limbs (36 patients) from the studied population. MUC was bilateral in 53% of patients. MUC type II was the most common subtype (74%), followed by types III and I

Minimal invasive posterior correction of Lenke 5C idiopathic scoliosis: comparative analysis of minimal invasive vs. open surgery

2019

Introduction Surgical management of adolescent idiopathic scoliosis in spite of usually favourable outcomes is still a major operation. Therefore, efforts are being undertaken to minimalize the procedure, reduce the surgical trauma and postoperative convalescence. The study was designed to compare posterior minimal invasive surgery using navigation based on intraoperative 3D imaging and standard open instrumented fusion in Lenke 5C idiopathic scoliosis treatment. Materials and methods From eight patients with Lenke 5C curves planned for posterior correction and instrumented fusion, four were treated with minimally invasive and four had open procedure. Operation length, estimated blood loss,…

Misdiagnosis of Thoracolumbar Posterior Ligamentous Complex Injuries and Use of Radiographic Parameter Correlations to Improve Detection Accuracy

2019

Study Design Retrospective study. Purpose To evaluate radiological parameters as indicators for posterior ligamentous complex (PLC) injuries in the case of limited availability of magnetic resonance imaging. Overview of Literature Traumatic thoracolumbar spinal fractures with PLC injuries can be misdiagnosed on X-rays or computed tomography scans. This study aimed to retrospectively assess unrecognized PLC injuries and evaluate radiographic parameters as indicators of PLC injuries requiring surgery. Methods In total, 314 patients with type A and type B2 fractures who underwent surgical treatment between 2001 and 2010 were included. The frequency of misdiagnosis was reassessed, and radiograp…

Fast-Track Surgery and Early Rehabilitation for Total Hip Replacement in Hospital of Traumatology and Orthopaedics

2019

Abstract Due to an ageing population, the necessity for hip replacement has grown, and therefore, new options are being sought, such as the Fast-track principle, to improve patient condition, reduce their hospital stay and enhance the hospital’s ability to treat more patients. The aim of this study was to investigate the effects of same-day patient mobilisation on pain, side effects, complications, duration of hospital stay, and recovery after primary hip replacement, using intermediate-acting local anaesthetics in spinal anaesthesia (SA). A prospective, randomised study was conducted at the Hospital of Traumatology and Orthopaedics. Forty-six patients undergoing total hip replacement were …

Reliability of isokinetic assessment of shoulder-rotator strength: a systematic review of the effect of position.

2011

Context:Isokinetic assessment of shoulder internal- (IR) and external-rotator (ER) strength is commonly used with many different postures (sitting, standing, or supine) and shoulder positions (frontal or scapular plane with 45° or 90° of abduction). Objective:To conduct a systematic review to determine the influence of position on the intersession reliability of the assessment of IR and ER isokinetic strength, to identify the most reliable position, and to determine which isokinetic variable appears to be most stable in intersession reliability.Evidence Acquisition:A systematic literature search through MEDLINE and Pascal Biomed databases was performed in October 2009. Criteria for inclusion…

Neuromuscular Training Warm-up Prevents Acute Noncontact Lower Extremity Injuries in Children’s Soccer : A Cluster Randomized Controlled Trial

2021

Background: Prevention of sports injuries is essential in youth, as injuries are associated with less future physical activity and thus greater all-cause morbidity. Purpose: To investigate whether a neuromuscular training warm-up operated by team coaches is effective in preventing acute lower extremity (LE) injuries in competitive U11-U14 soccer players. Study Design: Randomized controlled trial; Level of evidence, 1. Methods: Twenty top-level U11 to U14 soccer clubs in Finland were randomized into intervention and control groups and assessed for 20 weeks. Participants included 1403 players (280 female, 1123 male; age range, 9-14 years): 673 players (44 teams) in the intervention group and …

An Innovative Weightbearing Device for Weightbearing 3-Dimensional Imaging for Foot and Ankle Surgery Preoperative Planning.

2021

ABSTRACT Three-dimensional preoperative planning has demonstrated multiple surgical advantages. Currently, we cannot carry out preoperative 3-dimensional planning of foot and ankle orthopedics in most hospitals due to the impossibility of performing weightbearing CT imaging. Our objective is to describe and evaluate an innovative accessible, simple, and effective device that simulates standing while in a supine position, to obtain 3-dimensional images supporting bodyweight load with a conventional CT machine. From a group of 30 volunteers, 10 patients were randomly selected and pressure and its distribution were analyzed while in a standing position in both feet. Differences between both fe…

European guidelines on perioperative venous thromboembolism prophylaxis:Day surgery and fast-track surgery

2017

: In recent years, day surgery and fast-track surgery have experienced a continuous increase in volume. Many procedures are now performed on an outpatient protocol, including general, orthopaedic, oncological, reconstructive or vascular surgery. The management of these patients is safe, but the incidence of venous thromboembolism in this population remains unknown. Several risk factors can be identified and stratified derived from studies of inpatient surgical management (e.g. Caprini score). Recommendations for thromboprophylaxis should be tailored from the assessment of both personal and procedure-related risk factors, although with a lack of evidence for application in outpatient managem…

Three single loops enhance the biomechanical behavior of the transtibial pull-out technique for posterior meniscal root repair

2017

Purpose: To investigate the effect of applying an additional suture to enhance the biomechanical behavior of the suture–meniscus construct used during the transtibial pull- out repair technique. Methods: A total of 20 fresh-frozen porcine tibiae with intact medial menisci were used. In one half of all speci- mens (N = 10), two non-absorbable sutures were passed directly over the meniscal root from the tibia side of the meniscus to the femoral side (2SS). In other ten specimens, three sutures were passed over the meniscal root (3SS). All specimens were subjected to cyclic loading followed by load-to-failure testing. Displacement of the construct was recorded at 100, 500, and 1000 cycles. Fur…

Association between frontal plane knee control and lower extremity injuries : a prospective study on young team sport athletes

2017

Background/aimPoor frontal plane knee control can manifest as increased dynamic knee valgus during athletic tasks. The purpose of this study was to investigate the association between frontal plane knee control and the risk of acute lower extremity injuries. In addition, we wanted to study if the single-leg squat (SLS) test can be used as a screening tool to identify athletes with an increased injury risk.MethodsA total of 306 basketball and floorball players participated in the baseline SLS test and a 12-month injury registration follow-up. Acute lower extremity time-loss injuries were registered. Frontal plane knee projection angles (FPKPA) during the SLS were calculated using a two-dimen…
