Stridor in children

2018

A 16-month-old boy was referred to the emergency department of the Children's Clinical University Hospital (Riga, Latvia) due to cough and noisy breathing for 3 months. The complaints seemed to have worsened over time with coughing fits 2–4 days a week and the boy not being able to tolerate solid food (leading to vomiting) during the fits. In between the bouts, he felt fine. Diminished food intake was noted over the last month. Over the 3-month period, no other symptoms were noted. The father of the child was fixated on a diagnosis of asthma and categorically denied the possibility of any choking attacks after eating or playing with small objects. The boy had been seen by pulmonologist once…

Fever and dyspnoea in a tracheostomised patient

2020

A 65-year-old man was referred for evaluation of acute onset of fever, productive cough and dyspnoea. He had previously received a diagnosis of laryngeal carcinoma, which had been treated with laryngectomy and bilateral laterocervical lymphadenectomy, followed by chemotherapy. He underwent plastic surgery of the laryngocutaneous fistula, and a positron emission tomography (PET)- computed tomography (CT) examination performed during follow-up showed 18-FDG (2-fluoro-2-deoxyd- glucose) lung uptake in the apical right portion. He had a smoking history and his regular medications included dexamethasone, metoclopramide, omeprazole, furosemide, cholecalciferol and pregabalin. He had a history of …

A survey on sleep assessment methods

2018

Purpose A literature review is presented that aims to summarize and compare current methods to evaluate sleep. Methods Current sleep assessment methods have been classified according to different criteria; e.g., objective (polysomnography, actigraphy…) vs. subjective (sleep questionnaires, diaries…), contact vs. contactless devices, and need for medical assistance vs. self-assessment. A comparison of validation studies is carried out for each method, identifying their sensitivity and specificity reported in the literature. Finally, the state of the market has also been reviewed with respect to customers’ opinions about current sleep apps. Results A taxonomy that classifies the sleep detect…

Oleic Acid-Injection in Pigs As a Model for Acute Respiratory Distress Syndrome

2018

The acute respiratory distress syndrome is a relevant intensive care disease with an incidence ranging between 2.2% and 19% of intensive care unit patients. Despite treatment advances over the last decades, ARDS patients still suffer mortality rates between 35 and 40%. There is still a need for further research to improve the outcome of patients suffering from ARDS. One problem is that no single animal model can mimic the complex pathomechanism of the acute respiratory distress syndrome, but several models exist to study different parts of it. Oleic acid injection (OAI)-induced lung injury is a well-established model for studying ventilation strategies, lung mechanics and ventilation/perfus…
