Transanal total mesorectal excision for restorative coloproctectomy in an obese high-risk patient with colitis-associated carcinoma
2016
Transanal total mesorectal excision (TaTME) offers great potential for the treatment of malign and benign diseases. However, laparoscopic-assisted TaTME in ulcerative colitis has not been described in more than a handful of patients. We present a 47-year-old highly comorbid female patient with an ulcerative colitis-associated carcinoma of the ascending colon and steroid- refractory pancolitis. A two-stage restorative coloproctectomy including right-sided complete mesocolic excision was conducted. The second step consisted of a successful nerve-sparing TaTME and a handsewn ileal pouch-anal anastomosis. TaTME may extend the possible treatment options in inflammatory bowel disease, especially …
Advances in paediatric restorative dentistry
2009
Dental caries has been a highly prevalent and costly disease in the world, representing the most common infectious disease in the paediatric population. Caries lesions are also increasingly isolated in specific teeth and tooth morphology types, and there is growing controversy about the most appropriate restorative material for these lesions in deciduous teeth. In fact, restorative materials continuously improved in durability, strength, aesthetics, and anti-cariogenic properties. Moreover, the increasing demand for aesthetic restorative treatment has totally transformed the practice of paediatric dentistry in the last decade. Traditionally, the use of amalgam was a standard procedure in th…
The atraumatic restorative treatment approach: an "atraumatic" alternative.
2009
Introduction: Fear and anxiety are part of all human experiences and they may contribute directly to a patient?s behavior. The Atraumatic Restorative Treatment (ART) is a technique that may be an alternative approach in treating special care patients or those who suffer fear or anxiety. Objective: the aim of this paper is to review the ART technique as an alternative to reduce pain and fear during dental treatment. Materials and method: A search for the term ?atraumatic restorative treatment? was carried out in the MEDLINE search engine. References, from the last 10 years, containing at least one of the terms: ?psychological aspects?, ?discomfort?, ?fear?, ?anxiety? or ?pain?, were selected…
Anatomical basis for the interposition of a gastric pouch between the ileum and the anus after total proctocolectomy.
2003
After a total proctocolectomy, ileoanal continuity is achieved by an ileal pouch–anal anastomosis. This anastomosis is not possible when the ileum cannot reach the anus. To avoid definitive ileostomy in this circumstance, we devised a gastric pouch, taken from the left half of the vertical portion of the stomach, vascularized by the right gastroepiploic pedicle, then interposed it between the ileum and the anus. The aim of this anatomical study on seven cadavers was to estimate the capacity of this gastric pouch to reach the anus. The distance between the caudal edge of the pubic symphysis and the apex of the pouch was measured. It is accepted that an ileal pouch always reaches the anus wit…
