Background and Aims Small-bowel varices (SBVs) are an uncommon consequence of portal hypertension. Radiologic intervention is usually considered for first-line management. When radiologic intervention is not possible, management options become very limited. The aim of this study was to evaluate the usefulness of double-balloon enteroscopy (DBE)-facilitated cyanoacrylate-injection endotherapy of SBVs. Methods This was a retrospective review of DBE-facilitated cyanoacrylate-injection endotherapy of SBVs (December 2015 to October 2016). Balloon Enteroscopy: Single- and Double-Balloon Enteroscopy

Balloon enteroscopy is a method that allows endoscopic inspection of the entire small bowel, or large parts of it, while simultaneously making it possible to obtain histologic samples and carry out treatment measures. Studies of double-balloon enteroscopy (DBE) have confirmed the high diagnostic yield of the procedure, with an acceptably low complication rate (approximately 1% for diagnostic DBE and 3% to 4% for therapeutic DBE). The principal indication for the procedure is midgastrointestinal bleeding, that is, when the bleeding source is located in the small bowel. Entrepreneurship training for new ventures

Several studies in the field of international entrepreneurship have indicated the inadequacy of public policy support for new ventures and called for more tailored training programs. This paper introduces a training program created for new ventures in Central Finland and analyzes its benefits and shortcomings using a qualitative case study. The findings reveal that the training program provided useful knowledge on how to develop a business further and how to make it understandable and attractive to a funder. Abstract Based on a recent technical–economical analysis on the island of Pantelleria, a policy feasibility study for a complete upgrading of the energy system of this Mediterranean Island is carried out. In this paper the fundamentals of thermoeconomics for systems operated at variable load are discussed examining the cost formation process and separately the cost fractions related to capital depreciation (which require additional distinctions with respect to plants in steady operation) and to exergy consumption. The relevant effects of the efficiency penalty due to off design operation on the exergetic cost of internal flows are also examined. An original algorithm is proposed for the integrated optimization of plant design and operation based on an analytical solution by the Lagrange multipliers method and on a multi-objective decision function expressed either in terms of net cash flow or primary energy saving. The method is suitable for application in complex energy systems such as ‘‘facilities of components of a same product’’ connected to external networks for power or heat distribution. About 10 % of the mutations affecting the CFTR gene are "stop" mutations, which generate a Premature Termination Codon (PTC), thus resulting in the synthesis of a truncated CFTR protein.
