Aims Recanalisation rates of coronary chronic total occlusions (CTO) remain sub-optimal. The retrograde technique was recently introduced to improve success rates. Methods and results From February 2005 until December 2007, 175 patients were treated with this technique in seven European centres by highly experienced operators: in 84 (48%) as primary strategy, in 41 (23.5%) immediately after antegrade failure and in 50 (28.5%) as a repeat procedure after previous antegrade failure. Acute occlusion of the left main coronary artery (LMCA) is a rare angiographic finding. We report five patients with acute myocardial infarction (AMI) and one patient with unstable angina, in whom reperfusion was achieved or attempted with percutaneous transluminal coronary angioplasty (PTCA). All patients had a long history of stable angina pectoris. The indication for emergency PTCA was cardiogenic shock in the five patients with MI. PTCA was successful in five of six patients. Three patients with reperfusion survived, three died. All survivors underwent coronary artery bypass grafting (CABG) and were still alive at 23 months, 3 and 8 years respectively in NYHA functional class II or III.…

Combined medical and mechanical recanalization in acute myocardial infarction

A technique of combined medical and mechanical recanalization was employed in 96 patients with acute transmural myocardial infarction. The mean time between onset of symptoms and admission to hospital was 170 +/- 65 min (X +/- SD). After 10 +/- 16 min, 250,000 U streptokinase was administered intravenously for 20 min. Intracoronary thrombolysis was commenced within 38 +/- 14 min. First coronary angiograms demonstrated reperfusion, an open vessel in 25/96 patients (26%). In 15/71 patients (21%) reperfusion occurred during thrombolysis therapy, before mechanical recanalization could be performed. Takotsubo cardiomyopathy was described for the first time in Japan in the 1990s. It is very similar to the ischemic cardiopathy both for clinical and instrumental characteristics. His peculiarity is an alteration of the ventricular contraction mechanism with hypo-akinesis of the apex and lateral segments of the left ventricle, associated with hyper-kinesis of the heart base which is responsible for the typical echocardiographic aspect of a cruet during the systole. Secondary Circulation of Tropical Cyclones in Vertical Wind Shear: Lagrangian Diagnostic and Pathways of Environmental Interaction

Abstract This study introduces a Lagrangian diagnostic of the secondary circulation of tropical cyclones (TCs), here defined by those trajectories that contribute to latent heat release in the region of high inertial stability of the TC core. This definition accounts for prominent asymmetries and transient flow features. Trajectories are mapped from the three-dimensional physical space to the (two dimensional) entropy–temperature space. VERTICAL MOTION CHANGES RELATED TO NORTH-EAST BRAZIL RAINFALL VARIABILITY: A GCM SIMULATION

The atmospheric structure over north-east Brazil during anomalous rainfall years is studied in the 11 levels of the outputs of the Laboratoire de Meteorologie Dynamique atmospheric general circulation model (LMD AGCM). Seven 19-year simulations were performed using observed sea-surface temperature (SST) corresponding to the period 1970– 1988. The ensemble mean is calculated for each month of the period, leading to an ensemble-averaged simulation. The simulated March-April rainfall is in good agreement with observations. Synthesis, crystal structures, magnetic properties, and lattice dynamics of Ba2XCu(OH)[V2O7] with X=Cl, Br

Abstract We have synthesized Ba2XCu(OH)[V2O7] with X=Cl, Br by hydrothermal methods. The isotypic structures (Pnma, Z=4, a≈15.1 A, b≈6.1 A, c≈9.6 A) contain distorted hexagonal layers of Ba and X in a BN-type arrangement. Each halide is further coordinated by one out-of plane Ba atom in an alternate up-down fashion resulting in an overall Ba 2 X 3 + ∞ 2 structural feature. The planar Ba–X hexagonal rings are centered by divanadate groups in an eclipsed orientation. Edge-sharing chains of CuO 2/2 (OH) 2/2 O 2/1 5 - ∞ 1  complement the structure.
