Intravascular ultrasound imaging of arterial wall architecture.

Intravascular ultrasound (IVUS) is a promising new technique for assessing vascular morphology and structure. Controversy exists whether the three-layer appearance of the arterial wall correctly reflects the histologic structures of the intima, media, and adventitia. We performed an experimental study to clarify the three-layer appearance. The vessel wall architecture was analyzed by IVUS on eight different kinds of plastic cylinders, 24 normal blood vessels from pigs, and 59 human arterial segments. Biomechanics of human ascending aorta and aneurysm rupture risk assessment

Ascending aortic aneurysms (AsAA) and aortic dissections are life-threatening cardiovascular diseases. The main criteria for determining surgical intervention of AsAA are the maximum diameter or the increasing annual rate of the aneurysm. The mortality of the untreated aortic dissection can be 21% to 74%, depending on the delay of hospital admission.Our study aims to characterize the biomechanical properties of the ascending aorta and propose a patient-specific approach to assess the risks of rupture. Acute myocardial infarction, pulmonary embolism, and aortic dissection are diseases associated with acute chest pain and may lead to severe morbidity and mortality. These diseases may not be trivial to diagnose in the settings of emergency room. Radiologic and Intraoperative Finding of Intimal Tear in Type A Intramural Hematoma of the Aorta

AbstractAortic intramural hematoma (IMH) is described as “dissection without intimal tear” due to rhexis of vasa vasorum, which results in bleeding within the tunica media in the absence of intimal disruption or blood flow communication. The aim of our study is to validate perioperative evidence of intimal entry tear in IMH patients and to suggest that this entity may represent a part of a disease and not a separate disease.

Unusual Presentation of Bilateral Carotid Artery Dissection: The Importance of Reasoning Outside the Box

A bilateral internal carotid artery dissection presenting with atypical symptoms of cerebral hypoperfusion has been rarely reported, especially in the absence of obvious precipitating factors. A middle-aged woman presented to the emergency department with a 2-day-history of progressive left arm numbness and weakness, confusion, disorientation and clumsiness worsened by upright position. A cerebral hypoperfusion condition was hypothesized and confirmed by a CT angiography, which showed bilateral internal carotid dissection with uncertain etiology.
