Combining Biophysical Modeling and Machine Learning to Predict Location of Atrial Ectopic Triggers

2018

The search for focal ectopic activity in the atria triggered from non-standard regions can be time consuming. The use of body surface potential maps to plan the intervention can be helpful, but require an advance processing of the data, that usually involves to solve an ill-posed inverse problem. In addition, changes in maps due to pathological substrate such as fibrosis might affect the expected electrical patterns. In this work, we use a machine learning approach to relate ectopic focus activity in different atrial regions with body surface potential maps, and consider the effects of fibrosis in various densities and distributions. Results show that as fibrosis increases over 15% the syst…

Laparoscopic management of quadruplet heterotopic pregnancy after ovarian stimulation cycle

2016

Introduction. We report a rare case of a heterotopic abdominal pregnancy with three intrauterine pregnancy, one with a viable fetus, treated successfully by laparoscopy. Methods. A 40-year-old patient with primary sterility, who un- derwent pharmacological ovulation induction, presented abdominal pain in the right iliac region. A trans-vaginal ultrasound showed an image suggestive of ectopic gestation near the right ovary, associated with three intrauterine gestational sacs with only one viable fetus. Basing on this observation, we performed an operative laparoscopy that confirmed the presence of a right ectopic abdominal pregnancy. Since the right ampullar tubal was ruptured and bleeding, …

Does transvaginal color Doppler sonography differentiate between developing and involuting ectopic pregnancies?

1995

The objective of this study was to assess the addition of transvaginal color Doppler imaging to transvaginal ultrasonography and beta-human chorionic gonadotropin values to differentiate between developin and involuting ectopic pregnancies. Forty surgically confirmed ectopic pregnancies were classified into developing or involuting according to histopathologic findings. Results were compared retrospectively with the plasma beta-human chorionic gonadotropin values as well as the resistive index and pulsatility index obtained in corpora lutea and peritrophoblastic flow in serial examinations. Student's t-test was used for comparison of means. Logistic regression analysis was applied to predic…

Relevance of supraventricular runs detected after cerebral ischemia

2017

Objective:Prolonged ECG monitoring after stroke frequently reveals short paroxysmal atrial fibrillation (pAF) and supraventricular (SV) runs. The minimal duration of atrial fibrillation (AF) required to induce cardioembolism, the relevance of SV runs, and whether short pAF results from cerebral damage itself are currently being debated. We aimed to study the relevance of SV runs and short pAF detected by prolonged Holter ECG after cerebral ischemia during long-term follow-up.Methods:Analysis is from the prospective Find-AF trial (ISRCTN46104198). We included patients with acute cerebral ischemia. Those without AF on admission received 7-day Holter ECG monitoring. We differentiated patients …

Prognostic Role of Subsequent Atrial Tachycardias Occurring During Ablation of Persistent Atrial Fibrillation

2013

Background— The role of subsequent atrial tachycardias (AT) in the context of persistent atrial fibrillation (AF) remains undetermined. This study evaluated the prognostic role of subsequent ATs for arrhythmia recurrences after catheter ablation of persistent AF. Methods and Results— A total of 110 patients with persistent AF (63±9 years; 22 women; 61 long-lasting persistent AF) underwent pulmonary vein isolation followed by electrogram-guided ablation. After AF terminated to AT, patients were separated by the randomization protocol to receive either direct cardioversion (group A) or further ablation of subsequent ATs to sinus rhythm (group B). After a mean follow-up of 20.1±13.3 months af…

Immunohistochemistry Differentiates Papillary Thyroid Carcinoma Arising in Ectopic Thyroid Tissue from Secondary Lymph Node Metastases

2007

Objective: To verify whether immunohistochemistry might be useful in the distinction between a true laterocervical metastasis of an undetected thyroid carcinoma and a primary tumor outside the gland. Design: Galectin-3, cytokeratin 19, and HBME-1 were assessed in six cases (group A) of laterocervical masses harboring papillary thyroid carcinoma (PTC) but without a thyroid tumor, and in eight cases (group B) showing PTC both in the thyroid and in the laterocervical masses. In both groups, normal-looking follicles adjacent to the laterocervical neoplasia were present. Main outcome: We found that the apparently normal follicles in group A were negative for all the antibodies, while group B sho…

Non-invasive localization of atrial ectopic beats by using simulated body surface P-wave integral maps

2017

Non-invasive localization of continuous atrial ectopic beats remains a cornerstone for the treatment of atrial arrhythmias. The lack of accurate tools to guide electrophysiologists leads to an increase in the recurrence rate of ablation procedures. Existing approaches are based on the analysis of the P-waves main characteristics and the forward body surface potential maps (BSPMs) or on the inverse estimation of the electric activity of the heart from those BSPMs. These methods have not provided an efficient and systematic tool to localize ectopic triggers. In this work, we propose the use of machine learning techniques to spatially cluster and classify ectopic atrial foci into clearly diffe…

Ventricular paired pacing to control intractable junctional tachycardia following open heart surgery in a child.

1989

A 5-month-old girl presented postoperatively with an atrioventricular (A-V) junctional tachycardia at a rate of 245/min following surgical repair of tetralogy of Fallot. The systolic blood pressure dropped to 60 mmHg with this rapid heart rate, and the infant became shocked. Drugs and overdrive pacing were ineffective in suppressing the A-V junctional tachycardia and in improving cardiac output. Ventricular paired pacing was used successfully to halve the mechanically effective ventricular rate and to restore cardiac output. When ventricular paired pacing was stopped after 12 h, the spontaneous rhythm was an atrial rhythm with 1-1 A-V conduction. The patient was discharged in sinus rhythm o…

DECORIN EFFECTS ON PROTEOMIC PROFILING OF BREAST CANCER CELLS: AN UPDATED STUDY

2015

The malignant carcinomas are characterized by several capabilities acquired by the neoplastic cells, among which the ability to invade the extracellular matrix (ECM) and to establish a crosstalk with several ECM components. Under this respect, the extracellular microenvironment is an entity extraordinarily rich of information with opposite signals. Our group has long undertaken the study of the effects of ECM molecules on the behavior of cancer cells in vitro. Among the studied molecules, the decorin was found to exert a non-permissive effect on the growth and motility of the transfected tumor cells. The malignant carcinomas are characterized by several capabilities acquired by the neoplastic cells among which the ability to invade the extracellular matrix (ECM) and to establish a crosstalk with several ECM components. Under this respect the extracellular microenvironment is an entity extraordinarily rich of information with opposite signals. Our group has long undertaken the study of the effects of ECM molecules on the behavior of cancer cells in vitro. Among the studied molecules the decorin was found to exert a non-permissive effect on the growth and motility of the transfected tumor cells. The decorin belongs to the family of small leucine-rich proteoglycans (SLRP) and is involved physiologically in the fibrillogenesis of collagen. In the last few year a new anti-oncogenic role has been proposed for decorin1. This study aimed to implement the knowledge on the effects of ectopic decorin on breast cancer cells using as a reference point the results already achieved by our research group2 on the experimental model format. By breast cancer cell line 8701-BC and its transfected clone DEC-C2. The extension of the proteomic analysis combined with the mass spectrometry allowed to triplicate the number of identified proteins in our model. Among the newly identified proteins were members of the classes of metabolic enzymes S100 family and cell motility proteins which revealed a net decrease in the decorin transfected cells. Of considerable importance is the observation that these classes of proteins are the most involved in metastatic progression. These results confirm and reinforce the anti-oncogenic role hypothesized for decorin. The work was co-funded by the Italian 5x1000 to COBS.DECORIN
