Confocal laser endomicroscopy.

A miniaturized confocal microscope was developed that could be integrated in the distal tip of a conventional colonoscope. With this technique, denoted confocal endomicroscopy, subsurface analysis of the gut mucosa and in-vivo histology during ongoing endoscopy are possible in full resolution by point scanning laser analysis. The diagnostic spectrum of confocal endomicroscopy is expanding from screening and surveillance for colorectal cancer to Barrett's esophagus, Helicobacter pylori-associated gastritis, and gastric cancer. Evidence for extrinsic control of oesophageal primary peristalsis.

AbstractThe role of both peripheral and central mechanism in the control of primary peristalsis was studied in pigeon cervical oesophagus.The results from the transection of oesophageal muscular wall and of extrinsic nerves suggest that :i) primary peristalsis is programmed centrally.ii) extrinsic motor input is carried in glossopharyngeal nerves and distributed separately at each oesophageal level through intramural neurons.iii) intramural neurons do not seem capable of propagating the peristaltic sequence irrespective of the central control.

Helicobacter pylori and Barrett’s esophagus: a protective factor or a real cause?

Nothwithstanding the definite aethiopathogenetic path of certain diseases, the relationship between Helicobacter Pylori (H. Pylori) and barrett's esophagus (BE), a condition that increases the risk for dysplasia and consequently adenocarcinoma of the distal esophagus and esophagogastric junction, remains uncertain. this paper reviews the current scientific literature with emphasis on the protective correlation between H. Is Nasobiliary Tube Really Safe A Case Report

A case of esophageal ulcer caused by nasobiliary tube is described. This tool is not routinely considered to be a cause of major complications in the literature and to our knowledge, this is the first report of this kind of complication in nasobiliary tube placement. A 72-year-old patient presented with Charcot’s triad and was demonstrated to have cholangitis with multiple biliary stones in the common bile duct. Epidemiology and risk factors for oesophageal adenocarcinoma.

Oesophageal adenocarcinoma will soon cease to be a rare form of cancer for people born after 1940. In many Western countries, its incidence has increased more rapidly than other digestive cancers. Incidence started increasing in the Seventies in England and USA, 15 years later in Western Europe and Australia. The cumulative risk between the ages of 15 and 74 is particularly striking in the UK, with a tenfold increase in men and fivefold increase in women in little more than a single generation. Prognosis is poor with a 5-year relative survival rate of less than 10%. The advent of endoscopic resection (ER) techniques has enabled gastroenterologists to remove premalignant or neoplastic lesions throughout the gastrointestinal tract. Endoscopic Resection for High-Grade Dysplasia in Barrett’s Esophagus

Endoscopic resection (ER) has gained more and more importance in the treatment of high grade dysplasia (HGD) in Barrett's esophagus over the last few years. The choice of the different available techniques depends on the macroscopic type of the lesion and the personal experience of the endoscopist. But the "suck and cut"-technique with ligation device or cap should be favoured to normal strip biopsy in the esophagus because of the size of the resected specimen and its technical feasibility. Several studies demonstrate the effectivity and safety of ER in patients with HGD, therefore ER should be the treatment of choice in these patients.
