Cognitive Insight, Clinical Insight, and Reasoning in Schizophrenia : A Pilot Study in a Forensic Setting

This pilot study of 20 chronically ill male inpatients with schizophrenia and a history of violence investigates the relationships between cognitive insight, clinical insight, reasoning, and symptoms in a forensic setting. The majority (75%) of the patients with schizophrenia made hasty decisions based on a small amount of information (the jumping-to-conclusion bias, JTC). In addition, the data suggested that the more information patients gather, the more clinical insight they have and the less distressed they are by their symptoms. Una buona capacità di insight pare costituire un fattore predittivo di minore espressività psicopatologica nei pazienti affetti da psicosi. In questo lavoro intendiamo analizzare le relazioni tra grado di insight e gravità dei sintomi psicotici utilizzando i punteggi totali e parziali ottenuti alla Positive and Negative Syndrome Scale (PANSS) in un gruppo di pazienti affetti da psicosi all'esordio nell'ambito del progetto SGAP "Sicilian Genetic and Psychosis" svolto in collaborazione con il GAP Study dell'Institute of Psychiatry, King's College of London.

This research focused on factors associated with schizophrenia as well as on the feasibility and effectiveness of group metacognitive training (MCT) in schizophrenia patients with a history of violence in a high-security forensic hospital setting. One of the aims was to scrutinize reasoning ability in regards to jumping to conclusions (JTC) cognitive bias and the expression of cognitive insight (insight into one’s own thinking), a metacognitive ability.
