Pancreatic islet transplantation is a promising advanced therapy that has been used to treat patients suffering from diabetes type 1. Traditionally, pancreatic islets are infused via the portal vein, which is subsequently intended to engraft in the liver. Severe immunosuppressive treatments are necessary, however, to prevent rejection of the transplanted islets. Novel approaches therefore have focused on encapsulation of the islets in biomaterial implants which can protect the islets and offer an organ-like environment. Multipotential nestin and Isl-1 positive mesenchymal stem cells isolated from human pancreatic islets.

Mesenchymal cells in the developing pancreas express the neural stem cell marker nestin and the transcription factor islet-1 (Isl-1). Using defined culture conditions we isolated on a single cell basis nestin producing cells from human pancreatic islets. These cells were immortalized with lentiviral vectors coding for telomerase and mBmi. They are positive for Isl-1 and nestin and have the potential to adopt a pancreatic endocrine phenotype with expression of critical transcription factors including Ipf-1, Isl-1, Ngn-3, Pax4, Pax6, Nkx2.2, and Nkx6.1 as well as the islet hormones insulin, glucagon, and somatostatin. Pancreatic islet transplantation is emerging as a therapeutic approach for patients affected by diabetes. This approach consists of a minimally invasive procedure replacing insulin-producing cells (pancreatic islets). The technique has been proven successful, but limitations have been identified. Prolactin supplementation to culture medium improves beta-cell survival

We examined the effect of human recombinant PRL (rhPRL) supplementation to the culture media to determine its potential use in the context of clinical islet transplantation. Long-term function of porcine islets and single cells embedded in barium-alginate matrix.

The influence of alginate-embedding on the maintenance of functioning and morphological integrity in long-term culture of isolated porcine islets and islet cells was studied. Free-floating islets and islet cells served for control. Function was tested after the 1st, 2nd and 4th week. Basal and glucose-stimulated insulin secretion of embedded islets decreased slightly, but significantly after the first week (from 4.39 +/- 0.64 to 2.87 +/- 0.47 at normal and from 11.96 +/- 1.44 to 4.76 +/- 0.78 microU pro 24 h pro islet at elevated glucose concentration, p < 0.05 and < 0.01, resp.) and remained unchanged thereafter. Islet transplantation, since the 90’s, has been resulting to be one of the best successful example of human cell therapy. Nevertheless, islet isolation procedure is not completely standardized; in fact, more than fifty percent of islets procedures don’t arrive to their transplantation. Islet transplantation has become an accepted method to treat type 1 diabetes. To succeed and achieve normal levels of glucose in transplant recipients, the quality of the transplanted islets is of the utmost importance. Lack of oxygen during organ procurement, islet isolation, and subsequent culture triggers apoptosis or necrosis and loss of islet function, causing the yield and quality to diminish.
