[Letter to the Editor] Bipolar radiofrequency ablation for metastatic spinal tumors: an evolving paradigm in palliative care

I read with great interest the article by Wallace and col- laborators focusing on the combination of radiofrequency ablation (RFA) and vertebral augmentation for palliation of painful spinal metastases [1]. In a retrospective approach the authors reviewed a group of patients who underwent RFA for vertebral metastases in a period spanning almost 2 years. In a cohort of 110 patients, 72 RFA procedures were performed. Interestingly, 81 % of the metastatic lesions involved the posterior vertebral body and 45 % the pedicles. In almost all the cases, vertebral augmentation was performed following the ablation procedure.

Our group has long undertaken the study of the effects of ECM molecules on the behavior of cancer cells in vitro. Among the studied molecules the decorin was found to exert a non-permissive effect on the growth and motility of the transfected tumor cells. The decorin belongs to the family of small leucine-rich proteoglycans (SLRP) and is involved physiologically in the fibrillogenesis of collagen. In the last few year a new anti-oncogenic role has been proposed for decorin1. This study aimed to implement the knowledge on the effects of ectopic decorin on breast cancer cells using as a reference point the results already achieved by our research group2 on the experimental model format. By breast cancer cell line 8701-BC and its transfected clone DEC-C2. The extension of the proteomic analysis combined with the mass spectrometry allowed to triplicate the number of identified proteins in our model. Among the newly identified proteins were members of the classes of metabolic enzymes S100 family and cell motility proteins which revealed a net decrease in the decorin transfected cells. Of considerable importance is the observation that these classes of proteins are the most involved in metastatic progression. These results confirm and reinforce the anti-oncogenic role hypothesized for decorin. The work was co-funded by the Italian 5x1000 to COBS.DECORIN

Use of trastuzumab for breast cancer: the impact of age.

Indication for the use of trastuzumab was given in Italy in 2000 for the treatment of HER-2 metastatic breast cancer and in 2006 for early stage breast cancer. Information on trastuzumab use and on its possible variation with age in Italy is however limited. Using health care administrative databases, we evaluated the prevalence of the use of trastuzumab, and the probability for administration since the first hospitalization for breast cancer in various age groups, in two series of Italian women diagnosed with breast cancer in the Lombardy region (2004-2009) and in the Palermo district. Caspase-8 (casp-8) is well known as an initiator caspase involved in cell death signalling, although its activity in many cancer cell types seems to work under non-apoptotic conditions. Moreover, in several types of cancer, casp-8 is only rarely mutated and often its expression is very elevated. Despite the promising results of prostate-specific membrane antigen (PSMA)-targeted radioligand therapy (RLT) in metastatic castration-resistant prostate cancer (mCRPC), some patients show worsening disease during PSMA-RLT. We investigated the value of combined [18F]FDG and [68Ga]Ga-PSMA-11 PET imaging in this setting.
