Helical Tomotherapy® is a safe and feasible technique for total scalp irradiation

Angiosarcoma of the scalp is a rare aggressive tumor that affects elderly patients. Chemoradiation is the treatment of choice for multicentric and extensive disease. The shape of the scalp represents a dosimetric challenge in terms of achieving a homogeneous concave dose distribution with coverage of the entire target volume and an acceptable organs-at-risk sparing. We report a case of an 81-year-old man with a multifocal angiosarcoma of the scalp treated with Helical TomoTherapy® (Accuray Inc., Sunnyvale, CA, USA) intensity modulated radiotherapy. This technique allows precise and daily verifiable coverage of the target keeping the dose to the organs at risk within the constraints.

Aims To evaluate long-term late side effects, clinical and biochemical relapse in non-metastatic prostate cancer patients treated with dose escalation, from 74 to 78 Gy, by means of three dimensional conformal radiation therapy. Materials and Methods Clinical data of 125 patients with prostate cancer who underwent three-dimensional conformal radiation therapy were retrospectively evaluated. All patients were stratified, according to the NCCN classification, in low, intermediate and high risk, and all of them showed histologically proven adenocarcinoma stage T1–T3 with at least 2 years of follow-up. Sequential boost in neoadjuvant irradiation for T3N0-1 rectal cancer: long-term results from a single-center experience.

Purpose To evaluate the influence of radiation dose on tumor regression grade (TRG) and sphincter preservation rate in a series of cT3N0-1 rectal cancer patients treated with neoadjuvant chemoradiotherapy (CT-RT) with or without a sequential radiation boost. Materials and methods Between May 2002 and September 2013, 116 cases were eligible for retrospective evaluation. Radiotherapy was delivered for a total dose of 45 Gy (no boost arm) or 50.4 Gy (boost arm). TRG was evaluated with the Dworak scale. Results Median follow-up was 62 months (range, 12-138 months). To investigate the feasibility and tolerance in the use of adjuvant intensity modulated radiation therapy (IMRT) and simultaneous integrated boost in patients with a diagnosis of breast cancer after breast-conserving surgery.Between September 2011 to February 2013, 112 women with a diagnosis of early breast cancer (T1-2, N0-1, M0) were treated with IMRT and simultaneous integrated boost after breast-conserving surgery in our institution. Purpose: On dynamic IMRT plans with Millenium 80 (Varian) fields wider of 15 cm must be splitted. Since the EPIDdosimetry software does not allow to sum splitted subfluences in the same dosimetric portal image they have to be measured separately and compared independently with the subfluences previously calculated by the TPS Eclipse. FMECA Analyses of radiological over-exposure accident to patients in brachytherapy

This paper presents safety analyses of accidental events which can involve patient during High Dose Rate brachytherapy treatment in over-exposures. The study has been performed by using the well-known techniques FMECA modified by Fuzzy logic theory. Moreover, fuzzy HEART methodology was employed in order to evaluate human error probabilities for each treatment stage. The obtained results, aimed to obtain a list of the deviations with a reasonable probability to produce significant adverse outcomes, provided some recommendations for procedures and safety equipments to reduce the occurrence of radiological over-exposure accidents.
