Giant right atrial myxoma: A rare cause of cardiovascular collapse

We report the case of a patient with a giant right atrial myxoma that remained clinically silent until it almost completely obliterated the right atrium, prolapsed into the right ventricle and obstructed the tricuspid valve inflow. This case illustrates the importance of rapid surgical intervention in the setting of acute heart failure caused by tumor masses obliterating heart valves or cardiac chambers.

Recurrent On-Duty Sleepiness and Alertness Management Strategies in Long-Haul Airline Pilots

Introduction We examined whether long-haul airline pilots without recurrent on-duty sleepiness obtain more prior sleep and use more effective in-flight alertness management strategies than their colleagues with recurrent on-duty sleepiness. Methods There were 51 pilots who flew at least twice from Helsinki to Asia. Of them, 44 flew at least twice back to Helsinki following 1 local night. On-duty sleepiness was measured by the Karolinska Sleepiness Scale (KSS), alertness management strategies by a diary, and sleep by a diary and activity monitor. Pilots who rated KSS ≥ 7 on each, some, or none of the flights were classified as "regularly", "sometimes", and "never" sleepy, respectively. This …

A Bifurcation with Surprise

FFR was assessed after provisional stenting of a bifurcation stenosis in the proximal LAD in a very young patient. Pull-back demonstrated a good outcome after PCI, and it revealed the existence of an additional stenosis which, due to its very proximal location in the LAD, could not be seen at angiography. This case emphasizes the importance of performing FFR measurements during sustained hyperemia and using a slow pull-back technique.

Atenolol is a hydrophilic betareceptor blocking drug, which is predominantly eliminated via the kidneys, only about 5% of the atenolol is metabolised by the liver. After oral administration atenolol is incompletely absorbed from the intestine, so about 50% of the beta blocker are finally biovailable. In plasma only 3% of atenolol are protein-bound. We aimed to determine signs of nitrosative stress in serum/plasma of CHF patients. Several attempts have been done to define ACOS on the basis of medical history, symptoms, and functional findings.
