Abstract We submit a case report of a 78-year-old male came to our department for systolic heart, failure (EF of 25%). He has clinical history of recurring atrial tachycardia and atrial flutter previously treated in our department unsuccessfully with antiarrhythmic drug therapy. The echocardiographic evidence of left atrial enlargement (left atrium area 40 cm 2 ) and the clinical history of permanent atrial tachycardia has discouraged any attempt of substrate ablation. ARVD is part of the group of cardiomyopathies characterised pathologically by fibrofatty replacement of the right ventricular myocardium and clinically by right ventricular arrhythmias of the LBBB pattern. Pathogenesis, prevalence, and aetiology are yet not fully known. The diagnosis of ARVD is based on the presence of structural, histological, electrocardiographic, and genetic factors. Therapeutic options include antiarrhythmic medication, catheter ablation, implantable cardioverter defibrillation, and surgery. Isolated noncompaction of the ventricular myocardium (INVM) is an uncommon cardiomyopathy characterized by the persistence of fetal myocardium with prominent trabecular meshwork and deep intertrabecular recesses, often associated with systolic dysfunction and ventricular dilatation. Przedstawiamy pacjenta z nawracającymi niestabilnymi hemodynamicznie częstoskurczami komorowymi w okresie zawału mięśnia sercowego bez uniesienia odcinka ST powikłanego niewydolnością krążenia. Komorowe zaburzenia rytmu utrzymywały się pomimo wdrożonej farmakoterapii przeciwniewydolnościowej, zastosowania amiodaronu i przeprowadzonej rewaskularyzacji.
