Expression of inflammation-related genes in human atherosclerotic plaques

2010

Aims: Atherosclerosis is an inflammator y disease which molecular mechanisms are not been completely investigated. Our aim is to perform a wide expression study of inflammation related genes in human atherosclerotic plaques. Methods: The Human Inflammation Array (Applied Biosystems) was used to perform the mRNA quantification of 92 inflammation-related genes in 12 atherosclerotic plaques, their respective adjacent regions (with a lower grade lesion) and 7 healthy ar teries. The principle of the array is the real-time PCR amplification with a TaqMan probe specific for each gene. Data analysis was performed with SDS 2.3 software with the comparative Ct method using the gene beta-2-microglobulin as h…

The Role of Early Revascularization and Biomarkers in the Management of Diabetic Foot Ulcers: A Single Center Experience

2022

Diabetic neuropathy and Peripheral Arterial Disease (PAD) are the main etiological factors in foot ulceration. Herein, we report our experience of diabetic foot ulceration (DFU) management, with an analysis of the relationship between the rate of lower extremity amputation, in persons with infected DFU, after revascularization procedures performed to prevent major amputation. This study highlights the role of different biomarkers, showing their usefulness and potentiality in diabetic foot ulcer management, especially for the early diagnosis and therapy effectiveness monitoring. A retrospective analysis, from September 2016 to January 2021, of diabetic patients presenting diabetic foot with …

Endovascular Treatment with Drug-Eluting Balloon for Severe Subclavian Artery Stenosis Involving the Origin of the Vertebral Artery

2020

The first line approach for subclavian steal syndrome is PTA-stenting of subclavian artery. When the ipsilateral vertebral artery origin is involved or in closed proximity of the atherosclerotic lesion in the subclavian artery PTA-stenting is at risk of ipsilateral vertebral artery coverage. Herein we report our experience with DEB to address lesions involving the subclavian artery and the origin of the ipsilateral vertebral artery. From January 2017 to February 2019, patients presenting subclavian artery lesion involving the origin of the ipsilateral vertebral artery and treated using primary DEB, were included. Three patients, with left subclavian steal syndrome, were identified. The peri…

Carotid endarterectomy versus stenting in patients with contralateral carotid artery occlusion

2012

Aim. The aim of this prospective study was to compare outcomes after CEA and CAS in patients with contralateral carotid artery occlusion. Methods. Between 2004 and 2009, 527 consecutive patients underwent CEA (N.=281) or CAS (N.=246) for severe stenosis of internal carotid artery (ICA). Of them, 85 (16.1%) were identified with contralateral carotid artery occlusion. CEA was performed in 31(36.4%) patients with contralateral ICA occlusion, and 15 (48.4%) were symptomatic. Intraoperative shunts were placed in 12% versus 41.9% (P<0.001) patients with patent (N.=250) or occluded contralateral ICA (N.=31). Fifty-four (63.5%) patients with contralateral ICA occlusion underwent CAS with distal pro…

The Abdominal Compartment Syndrome (ACS) after Abdominal Aortic Aneurysm (AAA) open repair

2009

Objective: The abdominal compartment syndrome (ACS) is a ‘condition in which increased tissue pressure in a confined anatomic space, causes decreased blood flow leading to ischaemia and dysfunction and leading to permanent impairment of function’. Methods: Between June 2007 and June 2008 we treated surgically 23 cases of AAA (14 in election and nine in emergency), with indirect intra- abdominal pressure (IAP) monitoring (intra-vescical catheter). Mean age was 68 (64–84) years, 19 males and 4 females. Mean transverse diameter was 6.2 cm (5.5–9.0). Preoperative diagnostic procedure was ultrasound and tomography when possible. All patients were managed in hypotensive hemostasis (restricting fl…

Use of the directional atherectomy for the treatment of femoro-popliteal lesions in patients with critical lower limb ischemia

2016

Femoro-popliteal PTA for the treatment of critical limb ischemia is frequently associated with unsatisfactory procedural success rates while directional atherectomy (DCA) has improved success rate since claudicant patients undergoing percutaneous treatment of femoro-popliteal obstructive disease. The aim of this prospective study is to evaluate the safety, efficacy and procedural success of DCA, at one year, in the percutaneous treatment of femoro-popliteal obstructive disease in patients with critical limb ischemia.Methods. From March 2012 to March 2013 18 consecutive patients with critical limb ischemia were treated with DCA (Turbohawk/ Covidien-ev3 Endovascular Inc., North Plymouth, Minn…
