Family physician–patient relationship and frequent attendance of primary and specialist health care: Results from a German population-based cohort st…

2016

Abstract Objective To investigate the association between the quality of the family physician–patient relationship and frequent attendance of primary and specialist health care. Methods Cross-sectional survey of a representative German population sample ( N  = 2.266). Family physician–patient relationship was assessed with the Patient Doctor Relationship Questionnaire (PDRQ-9). Determinants of frequent attendance were analyzed using logistic regression. Results Frequent attendance of family physicians was associated with lower income (OR 1.43, 95% CI 1.02–2.00), not being in paid work (OR 1.58, CI 1.08–2.30), psychological distress (OR 1.14, CI 1.07–1.22), somatic symptoms (OR 1.07, CI 1.04…

Differential association of default mode network connectivity and rumination in healthy individuals and remitted MDD patients

2016

Rumination is associated with increased default-mode network (DMN) activity and functional connectivity (FC) in depressed and healthy individuals. In this study, we sought to examine the relationship between self-reported rumination and resting-state FC in the DMN and cognitive control networks in 25 remitted depressed patients and 25 matched healthy controls using independent component and seed-based analyses. We also explored potential group differences in the global pattern of resting-state FC. Healthy subjects with increased levels of rumination exhibited increased anterior DMN connectivity with the posterior DMN and the dorsal attention network and low connectivity within the anterior …

Depression and social phobia in essential tremor and Parkinson's disease

2017

Background Essential tremor (ET) and Parkinson's disease (PD) are the two most common movement disorders, and tremor is the most visible symptom. Comparative study on ET and PD clinical neuropsychiatric symptoms was performed to assess the impact of emotional state on tremor. Objectives To investigate the most common psychiatric symptoms (depression, anxiety and social phobia) and their correlations with motor symptoms, especially tremor, in ET and PD patients. Materials and Methods This comparative cross-sectional study consisted of neurological examinations, five self-assessment questionnaires (Depression Anxiety Stress Scale [DASS], Beck Depression Inventory [BDI], Social Interaction Anx…

Relation between Sexual Dysfunctions and Epilepsy, Type of Epilepsy, Type of Antiepileptic Drugs: A Prospective Study

2017

Introduction The aim of this study was to evaluate the incidence of sexual dysfunctions in males with epilepsy, the type of epilepsy, the frequency of seizures, the type of antiepileptic drugs (AEDs), the serum hormonal profile and the presence of psychiatric comorbidity. Methods Sixty-one patients focused on type of epilepsy, frequency of seizures, AEDs, hormonal profile and presence of mood disorders. We excluded all patients with severe neurologic and psychiatric impairment and patient who were not able to fill questionnaires. Mean age was 31.2 years (range 18-50 years); 31 patients (50.8%) had an idiopathic generalised epilepsy and 30 (49.2%) a focal epilepsy; among them, latter 18 (60%…

Psychiatric diagnosis in primary care patients with increased depressive symptoms.

2019

Screening of depression has been recommended in primary care and Beck's 21-item Depression Inventory (BDI-21) is a commonly used tool for screening. Depression has been shown to be frequently accompanied by comorbidities. This study aimed to analyze the characteristics, psychiatric diagnoses, and psychiatric comorbidity of primary care patients who have been screened for depression and referred to a depression nurse. The study subjects were primary care patients aged ≥ 35 years with depressive symptoms (BDI-21  9). Their psychiatric diagnosis were based on a diagnostic interview (Mini-International Neuropsychiatric Interview; M.I.N.I.) conducted by a trained study nurse.Of the 705 study subje…

A reanalysis of the center for epidemiological studies depression scale (CES-D) using non-parametric item response theory

2020

Abstract The “Center for Epidemiological Studies Depression Scale” (CES-D; Radloff, 1977 ) is a questionnaire used world-wide to measure depressive symptoms. Although the original four-factor-structure has been widely accepted and replicated, some studies point to other factor-structures like a one- and two-factor-structure. The goal of the current study was to evaluate the factor structure of the CES-D (one-, two- and four-factor-structure), which was found using classical test theory (CTT), with two non-parametric item-response-theory-models (Mokken-Scaling; Monotone-homogeneity-model; MHM and Double-monotonicity-model; DMM). To this end, a representative German sample was analyzed (N = 2…

Depression during an acute episode of schizophrenia or schizophreniform disorder and its impact on treatment response

2008

The aim of the present study was to examine the relevance of depressive symptoms during an acute schizophrenic episode for the prediction of treatment response. Two hundred inpatients who fulfilled DSM-IV criteria for schizophrenia or schizophreniform disorders were assessed at hospital admission and after 6 weeks of inpatient treatment using the Positive and Negative Syndrome Scale (PANSS) and the Hamilton Rating Scale for Depression (HAM-D). Depressive symptoms showed positive correlations with both positive and negative symptoms at admission and after 6 weeks, and decreased during 6 weeks of treatment. Pronounced depressive symptoms (HAM-D score> or =16) were found in 28% of the sample a…

Auditory hallucinations in first-episode psychosis: A voxel-based morphometry study

2019

Abstract Background Auditory hallucinations (AH) are a core symptom of psychosis. The brain abnormalities responsible for AH remain controversial due to inconsistent and conflicting findings across studies, with substantial confounding factors, such as chronicity. Few studies have examined the pathological changes that occur in the gray matter (GM) of patients with first-episode psychosis (FEP) and AH. The present study aims to validate the presence and characteristics of these structural abnormalities in relation to the intensity of psychotic symptoms and AH in a larger homogeneous sample than those of previous studies. Methods A magnetic resonance voxel-based morphometric analysis was app…

Vitamin D and clinical symptoms in First Episode Psychosis (FEP): A prospective cohort study

2019

BACKGROUND: There is a paucity of longitudinal research investigating vitamin D in people with early psychosis.METHOD: Vitamin D levels were measured in 168 patients (64% (n = 108) male, mean age 29.3 (9.8) years) with first episode psychosis (FEP), along with measures of clinical state at baseline and at 12 months follow up. We assessed the a) cross sectional, and; b) longitudinal relationships between continuous and categorical 25-hydroxyvitamin D (25(OH)D) levels and clinical symptoms at first contact for psychosis and at 12 months.RESULTS: In FEP, 80% (n = 134) at baseline, and 76% at 12 months follow up, had suboptimal vitamin D levels (<20 ng/ml). Suboptimal levels of 25 (OH) D at …
