Pulmonary microvascular architecture in hereditary haemorrhagic telangiectasia

2017

A 24-year-old Caucasian man was admitted with a known hereditary haemorrhagic telangiectasia (HHT) and heterozygous mutation of factor V Leiden following episodes of cerebral infarctions in occipital lobes, cerebellum and brainstem. In his case history, the patient underwent several interventional embolisation of arteriovenous (AV) malformations in the middle and lower lobes (figure 1). However, those were not completely successful as the malformations were diffuse. We performed video-assisted thoracoscopic surgery with a resection of the middle lobe and a wedge resection of segment 10. Figure 1 CT scans depict the pulmonary arteriovenous malformations after re-embolisation in the middle lo…

Reducing Rebreathing During Noninvasive Ventilation: Bias Flow or No Bias Flow?

2019

To the Editor : We read with interest the article by Signori et al[1][1] that evaluated CO2 rebreathing during noninvasive ventilation (NIV) via a full face mask connected to a double-limb ventilation circuit with 2 different configurations: (1) a single mask connector directly attached to the Y-

Material and Technology: Back to the Future for the Choice of Interface for Non-Invasive Ventilation – A Concise Review

2020

Non-invasive ventilation (NIV) has dramatically changed the treatment of both acute and chronic respiratory failure in the last 2 decades. The success of NIV is correlated to the application of the “best ingredients” of a patient’s “tailored recipe,” including the appropriate choice of the selected candidate, the ventilator setting, the interface, the expertise of the team, and the education of the caregiver. The choice of the interface is crucial for the success of NIV. Type (oral, nasal, nasal pillows, oronasal, hybrid mask, helmet), size, design, material and headgears may affect the patient’s comfort with respect to many aspects, such as air leaks, claustrophobia, skin erythema, eye irr…

Long-term volume-targeted pressure-controlled ventilation: sense or nonsense?

2017

The technology underlying the development of novel ventilatory modes for long-term noninvasive ventilation of patients with chronic hypercapnia is continuously evolving. Volume-targeted pressure-controlled ventilation is a hybrid ventilation mode designed to combine the advantages of conventional ventilation modes, while avoiding their drawbacks. However, manufacturers have created different names and have patented algorithms and set-up variables, which can result in confusion for physicians and respiratory therapists. In addition, clear evidence for the superiority of this novel mode has not yet been established. These factors have most likely hindered more widespread use of this mode in c…

A note on faithful traces on a von Neumann algebra

2009

In this short note we give some techniques for constructing, starting from a {\it sufficient} family $\mc F$ of semifinite or finite traces on a von Neumann algebra $\M$, a new trace which is faithful.

A characterization of regular circular languages generated by marked splicing systems

2009

AbstractSplicing systems are generative devices of formal languages, introduced by Head in 1987 to model biological phenomena on linear and circular DNA molecules. A splicing system is defined by giving an initial set I and a set R of rules. Some unanswered questions are related to the computational power of circular splicing systems. In particular, a still open question is to find a characterization of circular languages generated by finite circular splicing systems (i.e., circular splicing systems with both I and R finite sets). In this paper we introduce a special class of the latter systems named marked systems. We prove that a marked system S generates a regular circular language if an…

Congruence-based proofs of the recognizability theorems for free many-sorted algebras

2020

Abstract We generalize several recognizability theorems for free single-sorted algebras to free many-sorted algebras and provide, in a uniform way and without using either regular tree grammars or tree automata, purely algebraic proofs of them based on congruences.

Possible extensions of the noncommutative integral

2011

In this paper we will discuss the problem of extending a trace σ defined on a dense von Neumann subalgebra \(\mathfrak{M}\) of a topological *-algebra \({\mathfrak{A}}\) to some subspaces of \({\mathfrak{A}}\). In particular, we will prove that extensions of the trace σ that go beyond the space L1(σ) really exist and we will explicitly construct one of these extensions. We will continue the analysis undertaken in Bongiorno et al. (Rocky Mt. J. Math. 40(6):1745–1777, 2010) on the general problem of extending positive linear functionals on a *-algebra.

The Configuration of Space Through Architecture in the Thinking of Gadamer

2017

Although Gadamer stresses the importance of temporality, historicity and tradition, the aim of this contribution is to underline the uniqueness of architecture in Truth and Methods’ investigation relative to the essence of a work of art. The uniqueness of architecture for Gadamer lies not only in the fact that it gives space for the expression of all other kinds of artworks; a building has to be to understood as self-sufficient and as autarchic, but also as inscribed in the concrete historical life. A further aspect of the uniqueness of architecture originates in the fact that a building determines our way of life or our dwelling even in a political sense.
