Exercise for bone and cartilage in postmenopausal women with mild knee osteoarthritis

2016

The purpose of this study was to investigate the effects of high-impact exercise on bone mineral mass and strength, and on knee cartilage composition in postmenopausal women with mild knee osteoarthritis (OA). In addition, the association between knee OA and femoral neck bone structural characteristics in women with mild knee radiographic OA and those without radiographic knee OA was studied. Also, the reproducibility of measuring human knee joint cartilage by the delayed gadolinium-enhanced MRI (dGEMRIC) technique was determined in healthy asymptomatic subjects. Data from a 12-month randomized controlled trial (RCT) was used to assess the effects of exercise on bones and cartilages. The tr…

Emergence of severe spondyloarthropathy-related entheseal pathology following successful vedolizumab therapy for inflammatory bowel disease

2018

Objectives Vedolizumab (VDZ) blocks α4β7 integrin and is licenced for the treatment of IBD. It has been associated with mild SpA-related features, including sacroiliitis and synovitis. Herein we report a series of cases demonstrating the emergence of severe SpA-associated enthesitis/osteitis following successful IBD therapy with VDZ. Methods We evaluated 11 VDZ-treated patients with IBD across seven centres who developed severe active SpA and/or enthesopathy, with the aim of characterizing the VDZ-associated SpA or entheseal flares. Imaging features demonstrating particularly severe disease were recorded. Results De novo SpA developed in 9 of 11 patients and flare of known SpA in 2 patients…

Early Treatment of Systemic Juvenile Idiopathic Arthritis with Canakinumab and Complete Remission After 2 Years of Treatment Suspension: Case Report …

2019

Systemic juvenile idiopathic arthritis (sJIA) is an autoinflammatory disease characterised by fever and arthritis. We describe the case of a 14-year-old girl hospitalised with fever associated with rash, myalgia, arthralgia and polyarticular involvement. Examinations revealed increased levels of C-reactive protein, erythrocyte sedimentation rate, ferritin, triglycerides, leukocytes, neutrophils, lactate dehydrogenase, fibrinogen, aspartate aminotransferase (AST), alanine aminotransferase (ALT) and γ-glutamyl transferase (GGT). Bone marrow biopsy showed polyclonal leukocyte activation. A genetic study revealed a heterozygous mutation of the MEFV gene, c.442G>C (E148Q), which is typical of…

Dissecting the clinical heterogeneity of adult-onset Still's disease, results from a multi-dimensional characterisation and stratification

2021

Abstract Objectives To stratify adult-onset Still’s disease (AOSD) patients in distinct clinical subsets to be differently managed, by using a multi-dimensional characterization. Methods AOSD patients were evaluated by using a hierarchical unsupervised cluster analysis comprising age, laboratory markers systemic score and outcomes. The squared Euclidean distances between each pair of patients were calculated and put into a distance matrix, which served as the input clustering algorithm. Derived clusters were descriptively analysed for any possible difference. Results Four AOSD patients clusters were identified. Disease onset in cluster 1 was characterized by fever (100%), skin rash (92%) an…

Decreasing muscle performance associated with increasing disease activity in patients with rheumatoid arthritis

2018

Objectives Increasing evidence suggests that inflammation has a detrimental effect on muscle strength. Our objective was to analyse the association between muscle performance and different disease activity levels in patients with rheumatoid arthritis (RA). Method A total of 199 consecutive outpatients were subject to cross-sectional assessment. Measurements of grip strength, endurance of the upper and lower limbs and trunk strength were combined as a muscle performance composite score (MPCS), using a standardised method. The disease activity for 28 joints (DAS28), radiographs of small joints (Larsen score), rheumatoid factor, body mass index (BMI), comorbidities and anti-rheumatic drugs wer…

Interactive multiobjective optimization for finding the most preferred exercise therapy modality in knee osteoarthritis

2022

Abstract Background There are no explicit guidelines or tools available to support clinicians in selecting exercise therapy modalities according to the characteristics of individual patients despite the apparent need. Objective This study develops a methodology based on a novel multiobjective optimization model and examines its feasibility as a decision support tool to support healthcare professionals in comparing different modalities and identifying the most preferred one based on a patient’s needs. Methods Thirty-one exercise therapy modalities were considered from 21 randomized controlled trials. A novel interactive multiobjective optimization model was designed to characterize the effic…

Allelic variants of IL1R1 gene associate with severe hand osteoarthritis

2010

Background. In search for genes predisposing to osteoarthritis (OA), several genome wide scans have provided evidence for linkage on 2q. In this study we targeted a 470 kb region on 2q11.2 presenting the locus with most evidence for linkage to severe OA of distal interphalangeal joints (DIP) in our genome wide scan families. Methods. We genotyped 32 single nucleotide polymorphisms (SNPs) in this 470 kb region comprising six genes belonging to the interleukin 1 superfamily and monitored for association with individual SNPs and SNP haplotypes among severe familial hand OA cases (material extended from our previous linkage study; n = 134), unrelated end-stage bilateral primary knee OA cases (n…

Kuntoutuminen lonkan tekonivelleikkauksesta : kuntoutujien kertomat tarinamallit

2012

TIIVISTELMÄ Kuntoutuminen lonkan tekonivelleikkauksesta – kuntoutujien kertomat tarinamallit Knaapi-Junnila Sari Fysioterapian pro gradu-tutkielma. Jyväskylän yliopisto, liikunta- ja terveystieteiden tiedekunta, terveystieteiden laitos, 2012. 167 sivua, 36 liitesivua. Tämä tutkimus on osa Jyväskylän yliopiston johtamaa Osallistava potilasohjaus fysioterapiassa -tutkimushanketta. Tämän osatutkimuksen tavoitteena on ymmärtää kuntoutumisen merkityksiä lonkan tekonivelleikkauksesta kuntoutuville. Tutkimusaineisto koostuu kymmenen, 63–79-vuotiaan kuntoutujan haastatteluista leikkaukseen valmistautuessa ja siitä kuntoutumisen alkuvaiheessa. Tutkimuksessa selvitetään, minkälaisia tarinoita he kert…

Hybrid Hyaluronic Acid versus High Molecular Weight Hyaluronic Acid for the Treatment of Hip Osteoarthritis in Overweight/Obese Patients

2022

Background: Obesity is the main risk factor for hip osteoarthritis, negatively affecting the outcome of the disease. We evaluated the effectiveness of viscosupplementation with hybrid hyaluronic acid compared to that with high molecular weight hyaluronic acid in overweight/obese patients with hip osteoarthritis (OA). Methods: 80 patients were divided into two groups: a treatment group received two ultrasound-guided intra-articular hip injections of hybrid HA 15 days apart; a control group received a single ultrasound-guided infiltration with medium-high molecular weight hyaluronic acid (1500–2000 kDa). We assessed the pain, functional and cardiovascular capacity of the patients at bas…
