Cementless total hip replacement with subtrochanteric femoral shortening for severe developmental dysplasia of the hip.

Total joint replacement in patients suffering from developmental dysplasia of the hip poses specific technical difficulties due to insufficient bone stock at the site of the original and secondary acetabulum and a narrow, cranially displaced proximal femur. Twelve hips with severe congenital dislocation (4 Crowe type II, 5 type III, 3 type IV) were treated with cementless, porous structured total hip replacement. The cup was implanted at the anatomic height, a femoral segment was resected below the minor trochanter to reduce the femoral component in all cases. A ten-year follow-up of the Reflection cementless acetabular component.

We reviewed the long-term results at ten to 12 years of 118 total hip replacements in 109 patients using a second-generation hemispherical cementless acetabular component (Reflection) designed to address the problem of backside wear. Five patients (five hips) died and six patients (seven hips) were lost to follow-up. The remaining 98 patients (106 hips) had a mean age of 62.9 years (34.0 to 86.2) A rate of revision for aseptic loosening of 0.9%, and predictable results were found with respect to radiological evidence of fixation, lack of pain, walking ability, range of movement and function. Abstract Objective To evaluate the clinical results and survival of primary hip prosthesis with ceramic delta bearings (C-C) with a minimum follow-up of 5 years. Material and method A total of 205 primary hip arthroplasties performed between 2008 and 2012 were studied. The clinical results, pre-surgical and at 5 years of follow-up were evaluated using the Harris Hip Score (HHS), the Short Form-36 (SF-36), the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), and the visual analogue scale (VAS). The influence of lateral release on patello-femoral joint loading in knee arthroplasty

Eight fresh, cadaveric knees have been fitted with four different total knee cemented prostheses. Loading forces at the bone implant fixation interface of the patella component have been measured in a knee joint testing rig by four miniaturized force transducers, before and after a lateral release of the patellar retinaculum. The lateral release resulted in a significant local force reduction in the lateral and/or proximal quadrant of the patella resection plane depending on the knee flexion angle, quadriceps tensile force direction, and the patellar prosthesis design.

Nonadherence in outpatient thrombosis prophylaxis with low molecular weight heparins after major orthopaedic surgery

Background: According to some current guidelines, extended thromboprophylaxis after hip and knee arthroplasties is recommended. Outpatient prophylaxis with low molecular weight heparins (LMWH) is an important part of this prophylaxis, although the rates of adherence to these regimens is not known. Questions/purposes: We determined (1) the degree of nonadherence (NA) of patients with LMWH outpatient prophylaxis, and (2) whether specific independent factors explain NA.
