Primäre Ellenbogenprothese bei distalen Humerustrümmerfrakturen
2004
The use of hip- and shoulder arthroplasty in elderly patients for grossly displaced intraarticular fractures is being more and more accepted. Total elbow replacement (TER) has not been considered as an option for the treatment of extensively comminuted fractures of the distal humerus because surgeons feel the outcome is not predictable after TER generally. On the basis of a Medline literature research of this issue and the documented results based on the improvement of the designs of the implants, operative technique and the selection of patients, TER has a definite role in the treatment of some fractures of the distal humerus. We retrospectively reviewed forty-nine acute distal humeral fra…
The effects of physiotherapy using proprioceptive neuromuscular facilitation techniques on the gait of patients after hip and knee arthroplasty: a ca…
2021
Background: Gait disturbances are a major problem for patients after arthroplasty. After the surgery, walking speed, the rhythm of locomotion, and the length of the steps are significantly reduced. One of the therapeutic methods used in gait re-education is proprioceptive neuromuscular facilitation (PNF). Aim of the study: This study aimed to evaluate the effects of physiotherapy using PNF techniques on the gait of patients after hip and knee arthroplasty. Case report: A 60-year-old woman with advanced osteoarthritis that received bilateral hip and right knee arthroplasties was examined. Before and after the therapy, the following tests were performed: measurement of the range of motion (RO…
Total Elbow Replacement as Primary Treatment for Complex Fractures of the Distal Osteopenic Humerus
2003
Background: The use of hip and shoulder arthroplasty in elderly patients for grossly displaced intraarticular fractures is becoming more and more accepted. Total elbow replacement (TER) has not been considered an option for the treatment of extensively comminuted fractures of the distal humerus because surgeons feel the outcome is generally not predictable after TER. Is time to joint replacement a valid outcome measure in clinical trials of drugs for osteoarthritis?
2003
The rate of radiographic joint space narrowing is commonly used today as a structural outcome measure in clinical trials evaluating potential disease-modifying drugs in patients with hip osteoarthritis, but this results in a continuous variable. Among the methods proposed to circumvent this problem, it has been suggested that the incidence of total hip arthoplasty (THA) provides a hard outcome measure. It is a dichotomized variable, easy to measure and sensitive to change, with acceptable intrinsic validity. However, because this measure is limited by the variability of factors underlying the decision to perform surgery and the length of waiting lists, it has been suggested that time to ful…
Anterior dislocation in a total knee arthroplasty: A case report and literature review.
2015
Dislocations of a total knee arthroplasty (TKA) are an uncommon injury and only few cases of anterior dislocations have been reported. We report a rare case of anterior dislocation of a 10-year-old posterior stabilized total knee arthroplasty in a 74-year-old woman. The patient was successfully treated by close reduction of the dislocation followed by immobilization in full extension with a long leg cast. Because of a high risk of neurovascular complications, a high index of suspicion for vascular injury must be maintained. For this reason, a proper diagnosis is required and immediate reduction is recommended.
Aston Medical Duocentric® Reversed Prosthesis: Proposition of New Glenoid Component Design to Prevent Scapular Neck Notching
2015
Medialization of the center of rotation in reverse shoulder arthroplasty (RSA) results in greater excursion of the proximal humerus especially in adduction leading to a mechanical conflict against the neck of the scapula, thus creating a specific complication: scapular notching, in up to 90 % of RSA. We describe our third generation of RSA in Dijon, the Duocentric prosthesis. This prosthesis provides a new proposition to prevent the notching problem with an inferior overhang integrated to the glenoid baseplate. Such an inferior extension prevents the malpositioning of the hemisphere and resurfaces the “critical zone” preventing notching.Phenomenon while a smaller neck-shaft angle on the hum…
Postoperative Patient Education in Physiotherapy after Hip Arthroplasty : Patients' Perspective
2017
Objective The objective of the present study was to explore postoperative patient education in physiotherapy from total hip arthroplasty (THA) patients' perspectives, which has been a little‐studied area of research. The usefulness of rehabilitation protocols after THA has been debated over the years, and there are no clear guidelines for physiotherapy after THA Methods The data for the present qualitative study were collected from nine patients with THA by using individual interviews at the hospital after surgery. The interviews were audio‐recorded and analysed by using a phenomenographic method Results Four different categories of postoperative patient education in physiotherapy after hip…
Comparison of Postoperative Coronal Leg Alignment in Customized Individually Made and Conventional Total Knee Arthroplasty
2021
Neutral coronal leg alignment is known to be important for postoperative outcome in total knee arthroplasty (TKA). Customized individually made implants (CIM) instrumented with patient-specific cutting guides are an innovation aiming to increase the precision and reliability of implant positioning and reconstruction of leg alignment. We aimed to compare reconstruction of the hip–knee–ankle angle (HKA) of the novel CIM system iTotal™ CR G2 (ConforMIS Inc.) to a matched cohort of the off-the-shelf (OTS) knee replacement system Vanguard™ CR (Zimmer Biomet). Retrospective analysis of postoperative coronal full-leg weight-bearing radiographs of 562 TKA (283 CIM TKA, 279 OTS TKA) was conducted. V…
Kuntoutuminen lonkan tekonivelleikkauksesta : kuntoutujien kertomat tarinamallit
2012
TIIVISTELMÄ Kuntoutuminen lonkan tekonivelleikkauksesta – kuntoutujien kertomat tarinamallit Knaapi-Junnila Sari Fysioterapian pro gradu-tutkielma. Jyväskylän yliopisto, liikunta- ja terveystieteiden tiedekunta, terveystieteiden laitos, 2012. 167 sivua, 36 liitesivua. Tämä tutkimus on osa Jyväskylän yliopiston johtamaa Osallistava potilasohjaus fysioterapiassa -tutkimushanketta. Tämän osatutkimuksen tavoitteena on ymmärtää kuntoutumisen merkityksiä lonkan tekonivelleikkauksesta kuntoutuville. Tutkimusaineisto koostuu kymmenen, 63–79-vuotiaan kuntoutujan haastatteluista leikkaukseen valmistautuessa ja siitä kuntoutumisen alkuvaiheessa. Tutkimuksessa selvitetään, minkälaisia tarinoita he kert…
