The internal calcar septum and its contact with the virtual stem in THR: a computer tomographic evaluation.
2003
The internal calcar septum is a ridge of cortical bone protruding from the inner cortical wall of the proximal femur into the medullary canal. It extends from the lesser trochanter into the femoral neck and narrows the femoral cavity in its dorsal third. This region is essential for THR stability, but the degree of contact between the septum and standard THR implants has never been studied. We obtained CT scans of 50 arthrotic hip joints from patients requiring THR. Virtual stems (50 straight/wedge-shaped and 50 anatomic stems) were placed in CT images of the femora using a PC-based preoperative planning unit. The dimensions of the septum, degree and location of contact between the septum a…
Bilateral synovial chondromatosis of the temporomandibular joint.
2009
Abstract Purpose To report an exceptional case of bilateral synovial chondromatosis (SC) of the temporomandibular joint (TMJ) and discuss diagnostic approaches, treatment options and follow-up data. Patients and methods A 38-year-old woman presented with left preauricular swelling. Initial imaging studies revealed TMJ effusion only. Six years later, synovial calcifications were detected in the left TMJ; the right TMJ space was widened and presented incipient calcium deposits. Open arthrotomy of the left TMJ was performed, with removal of multiple cartilaginous loose bodies and complete synovectomy. Periodic controls proved the asynchronic development of intra-articular bodies in the right T…
Complement split products and proinflammatory cytokines in intraoperatively salvaged unwashed blood during hip replacement: comparison between hepari…
2008
Background and Objectives The aim of the present study was to investigate the quality of shed blood collected in a new intraoperative autotransfusion system (Sangvia®, AstraTech, Sweden) and to study whether heparin-coated surfaces in the device reduce the production of inflammatory mediators. Material and Methods The study was randomized and prospective. Twelve total hip arthroplasty patients whose blood was collected with a device having a heparin-coated surface and 12 patients whose blood was collected with a device having a non-heparin-coated surface were included. Venous blood was drawn from the patients preoperatively. Intraoperatively 200 ml salvaged blood was collected and samples…
Periacetabular Cortical and Cancellous Bone Mineral Density Loss After Press-Fit Cup Fixation
2012
The impact of total hip arthroplasty on strain adaptive bone remodeling has been extensively analyzed by dual-energy x-ray absorptiometry. In this study, we present a prospective computed tomography-assisted study of periacetabular cortical and cancellous bone mineral density (in milligrams of calcium hydroxyapatite [CaHA] per milliliter, or mgCaHA/mL) changes 10 days and 1, 3, and 7 years after press-fit cup implantation for 38 hips in vivo. Cancellous bone mineral density decreased by O -63% ventral and O -85% dorsal to the cup; cortical bone mineral density, by O -22% ventral and O -18% dorsal to the cup. The presented periacetabular strain adaptive bone mineral density data are the most…

Analysis of different geometrical features to achieve close-to-bone stiffness material properties in medical device: A feasibility numerical study
2021
Background and objective: In orthopedic medical devices, elasto-plastic behavior differences between bone and metallic materials could lead to mechanical issues at the bone-implant interface, as stress shielding. Those issue are mainly related to knee and hip arthroplasty, and they could be responsible for implant failure. To reduce mismatching-related adverse events between bone and prosthesis mechanical properties, modifying the implant's internal geometry varying the bulk stiffness and density could be the right approach. Therefore, this feasibility study aims to assess which in-body gap geometry improves, by reducing, the bulk stiffness. Methods: Using five finite element models, a unia…
In Silico Evaluation of Treatment of Periprosthetic Fractures in Elderly Patients After Hip Arthroplasty
2021
The aim of this study was to investigate the soundness of in silico finite element model (FEM) in the assessment of strain in the femur and in the components fixing the periprosthetic fracture in elderly patients after hip arthroplasty. From a group of 55 patients, aged 27–95, treated due to fractures after hip replacement in 2012–2018, 18 patients were separated out, aged over 85, out of whom 7 had type C fractures, according to the Vancouver classification. These seven patients formed the study group. The fractures were stabilized with a locking compression plate system and wire loops or by replacement of the endoprosthesis stem. The FEM was performed by processing radiological images of …
Biomechanical analysis of the humeral tray positioning in reverse shoulder arthroplasty design
2017
Despite the widespread use of reverse total shoulder arthroplasty, the fundamental effects of implant configuration on certain biomechanical outcomes have not been completely elucidated especially for the most innovative prostheses. Aim of this work is to investigate the behaviour of a new reverse shoulder prosthesis, characterized by a humeral tray with a variable offset, designed to increase the range of motion and to reduce the impingement. The purposes of this study were to evaluate the effect of reverse shoulder implant design parameters on the deltoid muscle forces, required to produce abduction, and on the shoulder range of motion, in order to provide a more systematic understanding …
