A Strange Case of Left Bowel Ischemia after Right Hernioplasty

We report the first observed case of a young man who suffered of large and unsuspected left bowel ischemia following an elective right open hernioplasty. A 54-year-old man had a 2-year history of right inguinal reducible mass and was admitted to hospital for an elective day case open inguinal hernioplasty for a direct right inguinal hernia. Apart from mild hypertension controlled with ACE inhibitor, he was medically fit and well. The patient was submitted to open tension-free mesh repair with polypropylene preshaped mesh with local infiltration anesthesia and additive sedation with midazolam. Atrial activity extraction for atrial fibrillation analysis using blind source separation.

This contribution addresses the extraction of atrial activity (AA) from real electrocardiogram (ECG) recordings of atrial fibrillation (AF). We show the appropriateness of independent component analysis (ICA) to tackle this biomedical challenge when regarded as a blind source separation (BSS) problem. ICA is a statistical tool able to reconstruct the unobservable independent sources of bioelectric activity which generate, through instantaneous linear mixing, a measurable set of signals. Combining Biophysical Modeling and Machine Learning to Predict Location of Atrial Ectopic Triggers

The search for focal ectopic activity in the atria triggered from non-standard regions can be time consuming. The use of body surface potential maps to plan the intervention can be helpful, but require an advance processing of the data, that usually involves to solve an ill-posed inverse problem. In addition, changes in maps due to pathological substrate such as fibrosis might affect the expected electrical patterns. Atrial fibrillation signatures on intracardiac electrograms identified by deep learning

BACKGROUND: Automatic detection of atrial fibrillation (AF) by cardiac devices is increasingly common yet sub-optimally groups AF, flutter or tachycardia (AT) together as ‘high rate events’. This may delay or misdirect therapy. OBJECTIVE: We hypothesized that deep learning (DL) can accurately classify AF from AT by revealing electrogram (EGM) signatures. METHODS: We studied 86 patients in whom the diagnosis of AF or AT was established at electrophysiological study (25 female, 65 ± 11 years). Paradoxical embolism after a femoral fracture

The foramen ovale is anatomically open in 25 % of individuals, but functionally closed by the higher pressure in the left antrum. Right-to-left shunt and subsequent paradoxical embolism may occur when pressure in the left antrum rises, for example, as a result of pulmonary embolism. In the present case we demonstrate a patient who presented 20 days after osteosynthetic treatment of a femoral fracture with word-finding deficits. Cerebral MRT revealed a fresh ischemic insult.
