Similarity and differences in elderly patients with fixed airflow obstruction by asthma and by chronic obstructive pulmonary disease

SummaryBackgroundEpidemiologic studies have demonstrated that elderly patients with fixed airflow obstruction can be affected by asthma or chronic obstructive pulmonary disease (COPD).MethodsWe studied 49 consecutive elderly outpatients, presenting fixed airflow obstruction, by clinical history (smoking), pulmonary function tests, blood gas analysis, and induced sputum.ResultsThe age was not different in patients with COPD (n=28) and asthma (n=21) (70.2±3.9 years vs. 69.6±3.7 years), also the degree of fixed airflow obstruction was similar (FEV1: 58.3±1.5% vs. 59.0±1.4% of predicted). Human sirtuin, an antiaging and antiinflammatory protein, is a metabolic NAD(+)-dependent protein/histone deacetylase that regulates proinflammatory mediators by deacetylating histone and nonhistone proteins. We thank Dr Vestbo for his interest in our manuscript “Diagnosis of airway obstruction in the elderly: contribution of the SARA study” and for the opportunity to clarify some points. As mentioned in the title and aims, the intent of the paper is to offer a contribution to the open issue of identification of airway obstruction in elderly people.1 We agree with Dr Vestbo about the utility of distinguishing between the clinical and functional aspects of chronic obstructive pulmonary disease (COPD). The severity grading systems proposed by the Global initiative for Chronic Obstructive Lung Disease (GOLD) have changed over time. Characterization and pilot scale fluidized bed gasification of herbaceous biomass: A case study on alfalfa pellets

Abstract Pilot-scale bubbling fluidized bed gasification tests of alfalfa pellets were performed at two different operational periods ranged on the basis of the two given equivalence ratio (ER) (the ratio between actual air and the stoichiometric air) namely as 0.25 and 0.30. During the test, the solid feeding rate was kept constant at 4.7 kg/h while the air input was varied and thus the ER. Increasing air from 4.16 to 4.99 N m3/h contributed to the evolution pattern of several parameters such as the rise in gas lower heating value (LHV) and gas yield, the average maximum of which were 4.2 MJ/N m3 and 1.5 N m3/kg respectively. Historically, asthma has been envisioned as a disease of younger ages. This has led to the assumption that respiratory symptoms suggestive of asthma occurring in older ages are to be attributed to conditions other than asthma, mainly COPD. Old observational reports and new epidemiological studies confirm that asthma is as frequent in older as it is in younger populations. Nevertheless, under-recognition, misdiagnosis and under-treatment are still relevant issues.
