In 19 patients with significant coronary artery disease, PTCR was used instead of PTCA. The PTCR was successful in all patients, but in nine patients additional PTCA was successfully performed. Coronary luminal narrowing could be reduced from 78 ± 19% to 32 ± 14% after rotablation and by combined treatment to 21 ± 14%. Only in one patient did coronary dissection occur; the patient was sent to surgery and had an uneventful outcome. No vessel perforation was observed. Computed tomography coronary angiography (CTCA) has reached very high standards both in terms of diagnostic performance and radiation dose reduction. This commentary follows a report on CTCA using less than 0.1 mSv in selected patients. This is an extraordinary accomplishment, both for technology and for medicine. The difficult task is now to implement this tool in clinical practice so it can play the best possible role. CTCA can improve diagnostic pathways, can save money for healthcare systems and could even improve pharmacological therapy. Myocardial Perfusion and Fractional Flow Reserve

Myocardial CT perfusion imaging is a rapidly evolving technology that allows assessment of the functional significance of potentially obstructive coronary stenosis. The opportunity for quantification of myocardial perfusion is the greatest potential advantage of CT compared with other perfusion techniques. On the other hand, fractional flow reserve during coronary angiography provides a functional assessment of coronary lesions identified with this modality. Recently, a non-invasive method for estimating fractional flow reserve, based on coronary CT angiography has been introduced. Nevertheless, at present neither CT perfusion nor CT FFR are ready for widespread use in clinical routine.

PURPOSE Evaluation of clinical relevance of coronary calcifications detected by helical CT. Evaluation of congruence with fluoroscopy (FS) and coronary angiography. Comparison with studies reporting on application of ultrafast computed tomography. MATERIALS AND METHODS 40 patients underwent helical CT (2 mm slice thickness, table-feed 3 mm/s). Coronary angiography and FS were performed in usual technique. Stenosis and calcifications were evaluated semiquantitatively. Heart Failure: Really Idiopathic?

Coronary angiography is necessary in the differential diagnosis of a newly discovered heart failure, which may be caused by multivessel disease and/or stenoses of major vessels. However, angiography is associated with a relatively high percentage of false negative findings in the assessment of left main disease. FFR allows unveiling these false negatives, even though special precautions need to be taken when stenoses are present also in the bifurcation branches.

We assessed the effect of intra-coronary attenuation on diagnostic accuracy using 64-slice computed tomography coronary angiography (CT-CA). We enrolled 170 patients with suspected coronary artery disease who underwent conventional coronary angiography (CA) and 64-slice CT-CA (100 ml of Iomeprol 400 mg I/ml at 4 ml/s). The study population was divided into two groups (85 patients each based on median attenuation of 326 HU) based on mean arterial attenuation; group 1 with low attenuation and group 2 with high attenuation. Relative incidence and mortality of congenital heart defects diagnosed by angiohemodynamic methods: a 17-year study.

Over a 17-year period (January 1971 to January 1988), 2322 children, aged 0-14 years, were diagnosed as having congenital heart disease (CHD) by cardiac catheterization and angiography. Excluding those with highly complex or undiagnosed defects, there were 2156 children with CHD, 72.4% of whom were treated surgically, with a total surgical mortality rate of 24.1%. After a mean follow-up of 9 years the overall mortality of the cohort was 29.9%, 29.1% occurring in the first month of life, 39.6% between 1 month and 1 year, and 31.2% between 1 and 14 years.
