Aneurysmal bone cyst of the mandible with conservative surgical management : a case report

The aneurysmal bone cyst is a nonneoplastic, osteolytic and locally destructive lesion that mainly affects the metaphyseal area of long bones and only 2% of it is diagnosed in the maxillofacial skeleton. Although surgical treatment is the most common option, it is associated to high morbidity rates. The case of an aneurysmal bone cyst of a considerable size in a 27-year-old male patient illustrating a conservative surgical approach with preservation of the dental structures in the mandible to limit aesthetic and functional side effects is presented. A laboratory examination revealed high immunoglobulin G (IgG) and IgG4 levels (5.95 g/L; normal range: 0.08-1.4 g/L), pointing to a diagnosis of systemic IgG4-related disease, with definite radiological evidence of biliary and pancreatic expression, and plausible renal, articular, salivary and lacrimal glands involvement. After a long period of neglect (Goodwin et al. 1955), percutaneous nephropyelostomy (PNS) has gained general acceptance and replaced operative nephrostomy. In addition, the percutaneous approach has led to a number of new diagnostic and therapeutic procedures. The objective of this review is not to give a complete and detailed survey of PNS but rather to report on our technique and results with fine needle nephrostomy and various endo-urological manipulations.
