[Objective]: To compare the rates of R0 resection in pancreatoduodenectomy (PD) for pancreatic and periampullary malignant tumors by means of standard (ST-PD) versus artery-first approach (AFA-PD). Usability of photoplethysmography method in estimation of conduit artery stiffness

Three channel photoplethysmography (PPG) signal waveform studies of leg conduit arteries during a provocative occlusion test were performed. PPG waveform second derivative amplitude ratio and arterial pulse wave velocity values showed significant correlations with ultrasound (US) reference method of local and regional arterial stiffness (AS), showing the ability to use PPG for AS change quantitative assessment.

PURPOSE This study evaluates the effects of pulmonary bolus injection of nonionic contrast medium on pulmonary artery pressure and resistance in patients with chronic thromboembolic pulmonary hypertension. METHODS In 39 patients (age 52 +/- 15) haemodynamic measurements were performed during bolus injection of nonionic contrast medium in a control group (I), in moderately severe (II) and severe pulmonary hypertension (III). RESULTS Initial inspiratory arrest caused significant pressure increase in all groups prior to bolus injection (delta PAsyst: 7.1 +/- 6.7 [I], 6.8 +/- 3.9 [II] und 7.2 +/- 7.9 mmHg p < 0.05). Objective To evaluate the hemodynamic effects of tolazoline and prostacyclin in infants with pulmonary vasospasm after cardiac surgery. Design Prospective cohort study. Setting Pediatric ICU. Patients The cohort consisted of 42 infants and children with congenital heart disease and pulmonary hypertension who underwent corrective surgery and were monitored postoperatively using pulmonary artery catheters. Fourteen infants (2 to 12 months old) in this group required postoperative treatment with tolazoline or prostacyclin. Sustained reentry in a 3d regionally ischemic human heart. A simulation study

In this work, we have studied the vulnerable window and propagation patterns in a human heart during acute ischemia. A 3-D biventricular model of a human heart with realistic heterogeneity and fiber orientations has been considered. The ischemic region was located in the anterior left ventricular wall mimicking the occlusion of the circumflex artery. The electrical activity of the tissue was modeled with the monodomain model along with a modified version of the ten Tusscher 2006 ionic model. Arteriovenous fistula of the vertebral artery in a female infant with hypotonia and cephalocorporal disproportion

Background:  Congenital arteriovenous fistulas are exceptional in childhood and imply a therapeutic challenge. Case report: A 9-month-old female infant was studied for cephalocorporal disproportion, hypotonia, progressive muscular atrophy and hyperreflexia. Computed tomography of the brain and electroencephalography were normal. Electromyographic patterns suggested proximal myopathic involvement. A continuous murmur with systolic reinforcement was audible in the neck. Optimal interventional treatment in a patient with occlusion of the brachiocephalic trunk and left subclavian artery with “double” steal syndrome

Subclavian steal syndrome, which comprises reversal of flow in the ipsilateral vertebral artery with stenosis or occlusion of the subclavian artery (SA) or brachiocephalic trunk (BCT), was first described by Contorni in 1960 [1]. This clinical syndrome manifests with the signs and symptoms of vertebrobasilar insufficiency, but can also be asymptomatic. This paper describes a step-by-step technique of complex endovascular treatment for a double steal syndrome caused by occlusions of the BCT and the left SA, associated with stenosis of the left internal carotid artery (ICA) and reversed flow in the left external carotid artery (ECA).

Reconstruction of Nasal Alar Defects with Freestyle Facial Artery Perforator Flaps

In 2009, we have described the use of freestyle facial artery perforator flaps for one-stage nose reconstruction. Since then, several articles have reported the use of facial artery perforator flaps for nose reconstruction. The purpose of this article is to provide an update of the published technique after 10 years of experience. Since 2004, 21 patients have been treated with a freestyle facial artery perforator flap for one-stage reconstruction of the nasal ala. The flaps were 16 propellers, 4 V-Y, and 1 island transposition. A single venous congestion leading to a minor flap tip necrosis and a wound dehiscence was observed.
