2017
Atherosclerotic cardiovascular disease still represents the leading cause of death in western countries. A wealth of scientific evidence demonstrates that increased blood cholesterol levels have a major impact on the outbreak and progression of atherosclerotic plaques. Moreover, several cholesterol-lowering pharmacological agents, including statins and ezetimibe, have proven effective in improving clinical outcomes. This document is focused on the clinical management of hypercholesterolemia and has been conceived by 16 Italian medical associations with the support of the Italian National Institute of Health. The authors have considered with particular attention the role of hypercholesterole…
Update on the role of the atherogenic lipoprotein phenotype in cardiovascular prevention
2009
Higher plasma triglyceride levels and decreased HDL-cholesterol concentrations are usually accompanied by the presence of small, dense LDL in the so-called lipid triad or ‘atherogenic lipoprotein phenotype’. This phenotype is highly atherogenic and its prevalence may suggest an even higher overall burden of atherosclerotic disease as compared with that associated with hypercholesterolemia. As stated by the National Cholesterol Education Program Adult Treatment Panel III, there is evidence suggesting each component of this lipid triad is individually atherogenic. However, the relative contribution of each component cannot be easily determined. Therefore, it has been suggested to consider th…
Vascular atherosclerotic disease: evaluation of the whole-blood filterability and red cell membrane microrheological pattern after intravenous admini…
1990
SummaryA study was carried out in 10 patients with vascular atherosclerotic disease to investigate the effects of a single dose of 300 mg pentoxifylline, given by intravenous infusion over 30 minutes, on various macrorheological and nzicrorheological parameters. Measurements were made at baseline, at the end of the intravenous infusion and again 30 minutes later of whole-blood filterability, erythrocyte membrane fluidity (using pyrene as probe) and transverse fluidity gradient (using fluorescent fatty acids as probes). The results showed that there was a significant variation in whole-blood filterability and the red cell membrane fluidity gradient after pentoxifylline. No difference, howeve…
Left Renal Vein Anatomical Anomalies: Radiological and Surgical Implications
1998
Objective: To emphasize the incidence and clinical, diagnostic and surgical implications of left renal vein developmental anomalies. Design: Retrospective analysis of case notes. Setting: Clinical Anatomy Department and Surgery Department, Faculty of Medicine, University of Valencia, Spain. Patients, participants: Standard radiological examinations (CT scan) performed on 745 patients; surgical findings at operation in 128 patients with aorto-iliac atherosclerotic disease, and 72 donor cadavers for transplantation. Main outcome measures: Incidence of left renal vein anomalies (retro-aortic left renal vein and peri-aortic renal venous collar), based on radiological (CT scan) and surgical obse…
Peroxisome Proliferator-Activated Receptors and Atherosclerosis
2011
The peroxisome proliferator-activated receptors (PPARs) represent the family of 3 nuclear receptor isoforms-PPARα, -γ, and -δ/β, which are encoded by different genes. As lipid sensors, they are primarily involved in regulation of lipid metabolism and subsequently in inflammation and atherosclerosis. Atherosclerosis considers accumulation of the cells and extracellular matrix in the vessel wall leading to the formation of atherosclerotic plaque, atherothrombosis, and other vascular complications. Besides existence of natural ligands for PPARs, their more potent synthetic ligands are fibrates and thiazolidindiones. Future investigations should now focus on the mechanisms of PPARs activation, …
