Decision making for solitary vestibular schwannoma and contralateral Meniere's disease.

The existence of dual inner ear pathology such as unilateral Meniere’s disease (MD) with a contralateral vestibular schwannoma (VS) is very rare, but provides the otologist with a significant management dilemma. In this study, we present 5 cases of unilateral disabling MD with a contralateral VS in the better hearing ear. Conservative management of the VS is mandated unless there are impending complications, with management directed toward controlling the vertigo attributed to MD. If and when the VS requires intervention, or the hearing in that ear deteriorates to unserviceable levels, cochlear implant of the ear affected by MD prior to addressing the VS provides optimal management.

Balance is a complex process that involves multiple sensory integrations. The auditory, visual, and vestibular systems are the main contributors. Loudness changes induced by a proximal sound: loudness enhancement, loudness recalibration, or both?

The effect of a forward masker on the loudness of a target tone in close temporal proximity was investigated. Loudness matches between a target and a comparison tone at the same frequency were obtained for a wide range of target and masker levels. Contrary to the hypothesis by Scharf, Buus, and Nieder [J. Acoust. Soc. Am. 112, 807-810 (2002)], these matches could not be explained by an effect of the masker on the comparison loudness, which was measured by loudness matches between the comparison and a fourth tone separated in frequency from the comparison and the masker. Multifrequency audiometry, using 20 Hz steps between 125 Hz to 800 Hz, studies patients affected by essential tinnitus. Bilateral normal auditive perception, tested using standard audiometers, was not confirmed in all patients analysed using the multifrequency technique. We observed neurosensorial deficit areas between 1040-1100 Hz and between 1920-1960 Hz. These results were integrated with diagnostic imaging and tested coclear nucleus microiscaemic areas and endococlear suffering. The patients who were given medical treatment showed benefits which were confirmed by partial regression of the neurosensorial deficit and tinnitus.

The aim of this work was to study the genotype distribution of Sicilian patients with biallelic GJB2 mutations; to correlate genotype classes and/or specific mutations of GJB2 gene (35delG-non-35delG) with audiologic profiles. A total of 10 different mutations and 11 different genotypes were evidenced in 73 SNHL subjects; 35delG (90.36 % of cases) and IVS1+1 (13.69 %) were the most common mutations found in the cohort with a significant difference in the distribution between North and South Sicily. Pure-tone thresholds and speech understanding were studied in three samples of men of different ages (31-35, 51-55 and 71-75 years). The air-conducted pure-tone thresholds were measured at 125-8,000 Hz and speech understanding by the help of interrupted and masked speech tests. The audiological measures were related to measures of other sensory functions, psychomotor speed, cognitive functions, psychic well-being, occupational and educational background and health.
