Postoperative maintenance therapy for inflammatory bowel disease.

2006

. Curr Opin Gastroenterol. 2006 Jul;22(4):377-81. Postoperative maintenance therapy for inflammatory bowel disease. Cottone M, Orlando A, Modesto I. Department of General Medicine, Pneumology and Nutrition Clinic, Palermo University, Palermo, Italy. PURPOSE OF REVIEW: This review will highlight the knowledge gained from studies published in the year 2005 on maintenance treatment after surgery for inflammatory bowel diseases. RECENT FINDINGS: In Crohn's disease the role of smoking in increasing the risk of relapse and recurrence after surgery is confirmed. Ornidazole seems effective in reducing endoscopic recurrence and clinical relapse after surgery. Probiotics do not appear to be effective…

An update on medical management on Crohn's disease.

2014

The management of Crohn's disease (CD) is continuously evolving. New issues emerging from more recent studies could influence the decision-making process in clinical practice. The aim of this review article is to highlight critical issues on the management of CD, new evidence from clinical trials, long-term prospective studies and real life experience, beyond the current guidelines. The role of mucosal healing in clinical practice is uncertain, clinical remission remains the primary end point. The timing for the definition of steroid-resistant CD should be considered between 2 and 4 weeks. Early treatment strategy with immunomodulators is effective for inducing remission but no controlled dat…

Randomised trial of mycophenolate mofetil versus azathioprine for treatment of chronic active Crohn’s disease

1999

BACKGROUNDCrohn’s disease is a chronic inflammatory disease of the alimentary tract. Azathioprine is an effective agent in the management of chronic active Crohn’s disease leading to long term remission of disease activity. Such treatment leads to limited efficacy or side effects in a small subset of patients.AIMSTo compare efficacy and side effects of treatment with azathioprine plus corticosteroids versus mycophenolate mofetil (MMF) plus corticosteroids in patients with chronic active Crohn’s disease.METHODSSeventy patients with chronic active Crohn’s disease (Crohn’s disease activity index (CDAI) greater than 150) were randomised for treatment with azathioprine/cortisone or MMF/cortisone…

Exacerbation of ulcerative colitis after rituximab salvage therapy

2007

Background: B-cells are considered to play a pathogenic role in human ulcerative colitis (UC) by producing autoantibodies that cause epithelial cell damage. Here we report on a patient with intractable UC who suffered from a severe exacerbation of UC after salvage therapy with rituximab, a B-cell-depleting anti-CD20-antibody. Methods: A 58-year-old patient with active long-standing UC and unresponsiveness or adverse events to mesalamine, corticosteroids, azathioprine, methotrexate, infliximab, leukapheresis, mycophenolate mofetil, and adalimumab received 375 mg/m2 rituximab. Results: A severe exacerbation of UC activity was noted upon therapy that required hospitalization. Subsequent studie…

Incidence of HSV and HPV with azathioprine

2009

IBd Incidence of HSV and HPV with azathioprine Mario Cottone and Sara Renna severe infections are an established risk of immunosuppressive therapy; however, the risk of opportunistic infections in patients with IBd who receive immunosuppressive therapy has so far only been studied retrospectively. the increased incidence of herpes flares and development or worsening of viral warts in patients with IBd who receive azathioprine has now been demonstrated for the first time in a prospective study. the rate of opportunistic infections in patients with iBD is dependent on the patient’s nutritional status, degree of innate immune system activity, whether the patient has undergone surgery and their…

Progressive multi-organ expression of immunoglobulin G4-related disease: A case report.

2012

A 63-year-old Caucasian man presented with a cholestatic syndrome, renal failure and arthralgias. A laboratory examination revealed high immunoglobulin G (IgG) and IgG4 levels (5.95 g/L; normal range: 0.08-1.4 g/L), pointing to a diagnosis of systemic IgG4-related disease, with definite radiological evidence of biliary and pancreatic expression, and plausible renal, articular, salivary and lacrimal glands involvement. Due to the rarity of the condition, there are currently no random control trials to point to the optimal therapeutic approach. The patient has been on steroid therapy with the subsequent introduction of azathioprine, with a complete resolution of all symptoms, a rapid reductio…

Comparing medical treatments for Crohn’s disease

2013

The drugs available for inflammatory bowel disease are aminosalicylates, antibiotics, steroids, immunosuppressors and biologics. The effectiveness of these drugs has been evaluated in many randomized clinical trials, mainly versus placebo. Few studies have been conducted comparing the different drugs among themselves, owing to the methodological problems raised by comparative trials, such as sample size and blindness. This review focuses mainly on the randomized clinical trials that have compared different treatments. Of course comparisons are mainly between drugs used in a particular setting (mild, moderate and severe disease). However, on many occasions there is no homogeneity in these cl…

Treatment of relapsing idiopathic nodular panniculitis (Pfeifer-Weber-Christian disease) with mycophenolate mofetil

1998

Idiopathic nodular panniculitis (Pfeifer-WeberChristian disease) is characterized by fever and symmetric subcutaneous nodules often located on the lower extremities.1,2 Histologically, a lobular panniculitis with a pronounced neutrophilic infiltrate is observed. The course of the disease is characterized by acute onset with fever and malaise and the simultaneous appearance of painful subcutaneous nodules. Some patients experience involvement of the intra-abdominal or retroperitoneal fat. Relapses are frequent, and a fatal outcome has been described repeatedly. Therapy usually consists of steroids in medium to high dosages, as well as immunosuppressive drugs such as azathioprine or methotrex…

Clinical and Morphologic Predictors of Outcome in a Multicenter Cohort of ITP Patients Treated with Trombopoietin Analogues

2018

Abstract Background: The role of bone marrow response in patients with immune thrombocytopenia (ITP) has gained paramount importance since the last 10 years, with the demonstrations that marrow megakaryocytes (MGK) are unable to properly compensate platelets peripheral destruction. TPO receptor agonists (TPOa), namely romiplostim (ROMI) and eltrombopag (EPAG), by stimulating megakaryopoiesis are able to induce a response in 74% to 94% of cases in clinical trials. However, real world use of these drugs has shown frequent changes in individual dose requirement, the possibility of treatment discontinuation, and their effectiveness outside registered indications; moreover, nothing is known abou…
