Endoscopic Resection for High-Grade Dysplasia in Barrett’s Esophagus

Endoscopic resection (ER) has gained more and more importance in the treatment of high grade dysplasia (HGD) in Barrett's esophagus over the last few years. The choice of the different available techniques depends on the macroscopic type of the lesion and the personal experience of the endoscopist. But the "suck and cut"-technique with ligation device or cap should be favoured to normal strip biopsy in the esophagus because of the size of the resected specimen and its technical feasibility. Several studies demonstrate the effectivity and safety of ER in patients with HGD, therefore ER should be the treatment of choice in these patients.

Chromoendoscopy and Magnifying Endoscopy in Patients with Gastroesophageal Reflux Disease

Gastroesophageal reflux disease (GERD) is common in the Western world. Upper endoscopy is needed to characterize the disease. Barrett’s esophagus as a complication of GERD is an established precancerous condition which can lead to adenocarcinoma in the distal esophagus. This review summarizes recent advances in the endoscopic characterization of Barrett’s esophagus using magnification endoscopy and chromoendoscopy. Methylene blue, indigo carmine and acetic acid are commonly used dyes to facilitate diagnosis of Barrett’s esophagus. miRNAs in the metaplastic transformation of the esophagus: something in the way toward carcinogenesis.

Background: Barrett's esophagus (BE) is a precancerous lesion that predisposes to development of esophageal adenocarcinoma (EAC) and it is associated with gastroesophageal reflux disease. Genetic changes involved in the metaplastic progression from squamous esophageal mucosa toward Barrett’s metaplasia and adenocarcinoma are almost unknown and there is an increasing interest in the developing of more objective biomarkers that can improve risk prediction. Several evidences suggest that some miRNAs are differentially expressed in the BE and EAC.
