The -308G/A polymorphism of TNF-alpha influences immunological parameters in old subjects affected by infectious diseases.

Abnormal increments of pro-inflammatory cytokines (IL-6 and TNF-alpha) characterize the outbreak of infectious diseases, which are the major cause of death in the elderly. A counterbalance to the inflammation is exerted by IL-10 with an inhibitory role on TNF-alpha production. As is well known, some cytokine gene polymorphisms influence the cytokine production, playing a role as susceptibility or resistance factors against immune-mediated and infectious disease. Erysipelothrix rhusiopathiae infections can typically be attributed to exposure to animal products; thus, these infections are more common among farmers, butchers and veterinarians, among others. This article describes a case involving a 54-year-old man who had relative neutropenia subsequent to treatment for chronic lymphocytic leukemia. The patient developed E rhusiopathiae bacteremia with concomitant pneumonia. The author of this article suggests a likely route of infection, which is atypical considering the patient did not report exposure to animal products.

Introduction One of the major advantage of procalcitonin compared to other parameters is its highly and early increase in response to bacterial infection. The increased procalcitonin levels can be observed 3 to 6 hours after infectious challenge.
