Capacity assessment for provision of quality sexual reproductive health and HIV-integrated services in Karamoja, Uganda

2020

Introduction: Sexual and reproductive health (SRH) and Human Immunodeficiency Virus (HIV) are crucial global health issues. Uganda continues to sustain a huge burden of HIV and AIDS.
Methods: A cross-sectional health facility-based assessment was performed in November and December 2016 in Karamoja Region, northern Uganda. All the 126 health facilities (HFs) in Karamoja, including 5 hospitals and 121 Health Centers (HCs), covering 51 sub-counties of the 7 districts were assessed. We assessed the capacity of a) leadership and governance, b) human resource, c) service delivery, d) SRH and HIV service integration and e) users satisfaction and perceptions.
 Results: 64% of the establi…

Once-daily tiotropium Respimat® 5 μg is an efficacious 24-h bronchodilator in adults with symptomatic asthma

2015

SummaryIntroductionOnce-daily tiotropium Respimat® 5 μg is an efficacious add-on therapy to inhaled corticosteroids (ICS) with or without long-acting β2-agonists in patients with symptomatic asthma. The objective of this study was to investigate whether the dosing regimen of tiotropium (once- versus twice-daily), delivered via the Respimat® SoftMist™ inhaler, affected 24-h bronchodilator efficacy and safety versus placebo Respimat® in patients with asthma who were symptomatic despite medium-dose ICS therapy.MethodsA randomised, double-blind, placebo-controlled, crossover study with 4-week treatment periods of tiotropium 5 μg (once-daily, evening) and 2.5 μg (twice-daily, morning and evening…

Early Lung Function Abnormalities in Acromegaly.

2014

BACKGROUND: Acromegaly is an insidious disorder caused by a pituitary growth hormone (GH)-secreting adenoma resulting in high circulating levels of GH and insulin-like growth factor I (IGF-I). Respiratory disorders are common complications in acromegaly, and can severely impact on quality of life, eventually affecting mortality. OBJECTIVES: The present study aimed to explore structural and functional lung alterations of acromegalic subjects. METHODS: We enrolled 10 consecutive patients (M/F: 5/5) affected by acromegaly. In all patients, magnetic resonance imaging (MRI) revealed the presence of pituitary tumor. All patients underwent clinical, lung functional, biological, and radiological as…

Type B Niemann-Pick Disease: Findings at Chest Radiography, Thin-Section CT, and Pulmonary Function Testing

2005

To evaluate findings at radiography, computed tomography (CT), and pulmonary function testing in patients with type B Niemann-Pick disease. The study was approved by the institutional review board or ethics committee at each study site and was compliant with HIPAA at the U.S. site. Written informed consent was obtained from each patient or guardian and minor assent was obtained from all children before any study-related procedures. Pulmonary involvement in 53 patients (27 male and 26 female patients; age range, 7-65 years; mean age, 23.3 years) with type B Niemann-Pick disease was evaluated with imaging and pulmonary function tests. All patients underwent chest radiography and thin-section C…

The influence of regular physical activity on lung function in paraplegic people

2015

Cross-sectional study. The main goal of this study was to examine the influence of regular physical activity (PA) on lung volumes and flows. The study was conducted in the Vall d’Hebron Hospital, Barcelona (Spain), and La Fe Hospital, Valencia (Spain). Spirometric tests were performed to 67 paraplegics, and differences were established between the active group (AG) (n=37) that performed >60 min per week of moderate-to-vigorous PA (MVPA) and 30 non-AG (NAG). Further, we established the relationship between the spirometric and PA variables and between being active and reaching the lower limit of normal (LLN) of the spirometric variables. AG had greater values than the NAG: FVC (P 0.05). The p…

HRCT and scleroderma: semiquantitative evaluation of lung damage and functional abnormalities.

2007

This study sought to validate the Warrick score in the assessment of lung involvement in systemic sclerosis (SSc) and correlate the results with functional abnormalities. In addition, we propose the use of high resolution computed tomography (HRCT) indices of alveolitis and fibrosis to discriminate between different stages of the disease.Thirty-one patients with SSc (16 with the diffuse form and 15 with the limited form) underwent functional and HRCT evaluations of the lung. The semiquantitative evaluation of radiological involvement, as proposed by Warrick, provides a score for each lesion based on the severity and the extent of the pulmonary damage. In addition to the total score, indices…

Lomitapide affects HDL composition and function

2016

Abstract Background Lomitapide reduces low-density lipoprotein-cholesterol (LDL-C) but also high-density lipoprotein-cholesterol (HDL-C) levels. The latter may reduce the clinical efficacy of lomitapide. We investigated the effect of lomitapide on HDL-C levels and on cholesterol efflux capacity (CEC) of HDL in patients with homozygous familial hypercholesterolemia (HoFH). Methods and results Four HoFH patients were treated with increasing dosages of lomitapide. Lomitapide decreased LDL-C (range −34 to −89%). Total HDL-C levels decreased (range −16 to −34%) with a shift to buoyant HDL. ABCA1-mediated CEC decreased in all patients (range −39 to −99%). The changes of total, ABCG1- and SR-BI-me…

Resilience and Coping as Predictors of Well-Being in Adults.

2016

Well-being is one of the keys to successful and optimal development across the lifespan. Based on the idea that development involves changes in individuals' adaptive capacity to meet their needs over time, the changes that occur in the second half of life require effort to adapt to the new reality. This study used a structural model to test the effects of coping strategies and resilience on well-being in a sample of 305 mid-life adults. Several constructs were measured: coping strategies, resilience, and well-being. A final model was obtained with good fit indices; psychological well-being was positively predicted by resilience and negatively by emotional coping. Moreover, positive reapprai…

Effects of allergen exposure on methacholine and AMP-induced air trapping in pollen-sensitive subjects

2011

Summary Background The effect of pro-inflammatory stimuli on bronchoconstrictor-induced air trapping has not been studied. Objective To determine the effect of natural allergen exposure, a pro-inflammatory stimulus, on methacholine- and adenosine 5′-monophospate (AMP)-induced air trapping. Methods Airway responsiveness to methacholine and AMP before and during the pollen season was obtained in 25 subjects with pollen allergy and in 10 healthy controls. The response was expressed by the sensitivity (PC 20 value) and by the slope and intercept of the FVC values recorded at each step of the challenge against the corresponding FEV 1 values. Results The slope and intercept FVC versus FEV 1 value…

The effects of instrumental reminiscence on resilience and coping in elderly

2014

A B S T R A C T Objectives: Aging, as a stage of development is marked by major changes to which the subject must adapt. Instrumental reminiscence is based on recalling times one coped with stressful circumstances, and analyzing what it took to adapt in those situations. The purpose of the present study was to analyze the effectiveness of an instrumental reminiscent program to enhance adaptive capacity (problemfocused coping and emotion-focused) and resilience in older adults. Method: Thirty participants noninstitutionalized conducted a pre and post assessment on a treatment consisting of the Mini-Mental State Examination (MMSE), Brief Resilient Coping Scale and Stress Coping Questionnaire …
